迷走神经切断对内毒素血症大鼠肺部炎症反应影响的初步研究

摘  要:目的 :探讨切断双侧颈部迷走神经对静注内毒素 (主要成分脂多糖Lipopolysaccharide,LPS)引起的大鼠肺部急性炎性损伤的影响。方法 :采用静注LPS(10mg/kg)引起大鼠急性肺脏炎性损伤的模型 ,分别切断双侧颈部迷走神经干或做假手术对照 ,在 1.5h检测各组动物血乙酰胆碱 (ACh)、皮质醇水平以及肺组织中肿瘤坏死因子 -α(TNF -α)、丙二醛 (MDA)量和髓过氧化物酶 (MPO)活性的变化。结果 :静注LPS1.5h即可引起肺部急性炎性病理改变 ,肺组织TNF -α、MDA量和MPO活性明显升高 (P <0 .0 1) ,血ACh和皮质醇也升高 (P<0 .0 5 ,P <0 .0 1)。静注LPS复合迷走神经切断组动物较单纯静注LPS动物肺组织中TNF -α、MDA量和MPO活性显著升高 (P <0 .0 5 ) ,肺组织炎性改变加重 ,而血ACh和皮质醇水平却显著下降 (P <0 .0 1)。结论 :迷走神经切断能加重肺组织的炎症反应和病理损害 ,迷走神经的完整性对于维持体内抗炎激素的水平 ,保护肺脏免受过度炎症的损害具有重要意义。Objective:To determine the effects of bilateral cervical vagotomy on the inflammatory reaction in lung tissue and the acute lung injury in endotoxemia rats.Methods:Animals were divided into three groups. Goup Ⅰ was performed as sham operation, group Ⅱ were injected LPS intravenously (10mg/kg ,Ecoli O111 :B4,Sigma),and group Ⅲ were subjected to bilateral cervical vagotomy following administration of LPS. Blood and tissue samples were collected at 1.5h after LPS administration, and TNF-α, MPO , MDA in lung tissue and acetylcholine , corticosterone in serum were determined. Results:Administration of LPS resulted in an elevation in levels of acetylcholine , corticosterone in serum and TNF-α , MPO , MDA in lung tissue. The inflammatory reaction was also discovered in lung tissue. Changes in parameters above and pathologic lesion in animals with endotoxemia were much more aggravated after vagotomy, but the contents of corticosterone and acetylcholine in serum were significantly decreased. Conclusions:Vagotomy can aggravate the inflammatory reaction and pathologic lesion in lung tissue,which suggests that vagus nerve play a very important role in regulating the anti-inflammatory hormones and protecting lung tissue from super-inflammatory injury.
