摘 要:目的 检测肿瘤坏死因子-α(TNF-α)血浆浓度变化,探讨TNF-α与精神分裂症的关系。方法 将同胞中有2名及2名以上符合ICD-10精神分裂症诊断标准的患者的51个家系的222人纳入研究,其中父母组94人,患病同胞组98人,非患病同胞组30人,同时纳入非血亲正常对照组40人。用酶联免疫吸附法(ELISA)对TNF-α血浆浓度进行测定,比较各组血浆浓度变化。结果 (1)4组间血浆TNF-α浓度(pg/ml)比较差异存在显著性(F=4.595,P=0.004),患病同胞组血浆TNF-α浓度(62.01±35.01)分别低于非患病同胞组(92.02±43.78)、非血亲正常对照组(82.94±41.62)及父母组(77.23±35.05),差异均有显著性(P=0.001,P=0.01,P=0.016)。(2)在病人组中,按是否服药(服药组:69.11±37.29;未服药组:67.01±36.36)、性别(男:71.64±40.70;女:65.06±32.99)分别分组比较中,差异均无显著性(P>0.05);年龄与血浆TNF-α浓度之间无明显相关性(r=0.037,P=0.717),病程长短与血浆TNF-α浓度之间也无明显相关性(r=0.104,P=0.306)。结论 精神分裂症患者血浆TNF-α浓度低于正常,年龄、病程、抗精神病药物、性别对精神分裂症血浆TNF-α浓度没有明显影响,提示精神分裂症患者免疫功能降低,支持免疫功能异常假说。Objective To investigate the relationship between schizophrenia and tumor necrosis factor-alpha (TNF-α). Methods The subjects were 51 pedigrees, and each pedigree must have at least two siblings schizophrenic patients who were up to the ICD-10 diagnose criteria of schizophrenia, including 94 parents, 98 sick siblings and 30 siblings without schizophrenia. In addition, 40 normal controls were involved. The plasma levels of TNF-α were measured by enzyme linked immunosorbent assay (ELISA). The plasma levels were compared by one-way ANOVA, Independent Samples t Test and Correlation Analyses. Results The mean plasma levels of TNF-α(pg/ml) in the four groups : sick siblings group (62. 01±35. 01), siblings without schizophrenia group (92. 02±43. 78), parents group (77. 23±35.03) and normal controls group (82. 94±41. 62). There were significant differences among these four groups (F=4. 595, P = 0. 004). The plasma levels of TNF-α in sick siblings group were significantly lower than those in siblings without schizophrenia group (P = 0. 007), parents group (P = 0. 016) and controls group (P = 0. 01) respectively. In sick siblings group, they were divided into subgroups according to administration and sex, and compared with each other. There was no significant difference between administration groups and between sex groups respectively. The age was not significantly relevant to the plasma levels of TNF-α and the duration was not too. Conclusions The plasma levels of TNF-α in schizophrenia were lower than those of normal controls. Age, duration, administration and sex had no evident influence on schizophrenia. These suggested that the immune-function in schizophrenia had weakened and supported the hypothesis of immune dysfunction of schizophrenia.
