视神经萎缩的固视状态对图型视觉诱发电位检查方法的影响

摘　　要：目的 :分析和探讨视神经萎缩时患眼视网膜偏心度对图型视觉诱发电位 (P -VEP)检查方法的影响。方法 :3 1例 5 3眼视神经萎缩患者接受了非矫正的P -VEP检查 ,患眼最佳矫正视力高于 0 0 2。全部患眼按照视力性质分为 0～10°组 (全部患者 )和 0～ 3°组 (低偏心度患者 ,3 9眼 )。结果 :0～ 3°和 0～ 10°两组均偏心度和VEP潜时间正相关关系明显 (P <0 0 5 )。直线回归分析表明 :两组均回归关系明显 ,方差分析意义显著 (P <0 0 5 )。但两组的回归系数分别为 :0～ 3°组 9 11,0～ 10°组为 2 42。结论 :结果表明随患眼固视的视网膜偏心度增大 ,患眼的P -VEP潜时均有相应延长 ,但两组间延长程度不同 ,0～ 3°组延长明显。提示在偏心视力患眼出现的情况下 ,常规VEP检查方式在某种程度上减弱或部分掩盖了病变的损伤和影响程度 ,应该进行矫正VEP检查。即在偏心视力患眼 ,视网膜偏心度对视觉诱发电位 (VEP)的影响不仅表现为对VEP检查结果的影响 ,而且也对常规VEP检查方式产生了影响。Objective:To analyze and evaluate the impacts of retinal eccentricity on the examination procedure of P-VEP in optic atrophy(OA). Methods:31 patients(53 eyes)in different retinal eccentricity with optic atrophy underwent ordinary P-VEP without the correction of eccentric fixation.All the eyes were divided into 2 groups:0～10° group(all the eyes)and 0～3° group(39 eyes). Results:The postitive correlation and linear regression between eccentricity and P-VEP latency in both the 2 groups were significant(P<0.05).But the coefficent of regression in 0～3° group was greater than that in 0～10° group. Conclusions:The eccentricity of the OA eye would not only affect P-VEP latency but also the P-VEP examination procedure.Ordinary P-VEP procedure might decrease the latency delay and,therefore,the ordinary P-VEP procedure should be corrected.
