常用两种单侧唇裂整复术对患者上唇和鼻部影响的比较

摘　　要：目的　讨论两种唇裂修复方法对各种单侧唇裂患者唇鼻畸形的影响。方法　选择符合遴选条件的单侧唇裂患者 (Ⅱ、Ⅲ度各 2 1人 ) ,精确测量患者手术前后唇鼻部健患侧的唇高度、唇高差、侧唇高、鼻孔宽度和高度、唇峰-口角距等项内容。测得相关数据进行统计学分析。结果　①不论是Ⅲ度还是Ⅱ度单侧唇裂 ,Tennison法对患侧鼻孔高和唇高间差异有统计学意义 (P <0 .0 5 )。②不论是Ⅲ度还是Ⅱ度单侧唇裂 ,Millard法对患侧鼻孔宽和高间差异有统计学意义 (P <0 .0 5 ) ,但对健 -患侧唇高差则只对Ⅱ度唇裂间差异有统计学意义 (P <0 .0 5 )。结论　对于鼻畸形较重的Ⅱ度唇裂患者应尽量采用Millard法唇裂整复术 ;对于鼻畸形较轻的Ⅲ度唇裂患者应尽量采用Tenni son法唇裂整复术 ;而对于鼻唇畸形均严重的Ⅲ度唇裂患者可采用Tennison法唇裂整复术。Objective To compare the different influences between Tennison lip repair and Millard lip repair on nasolabial region of different type unilateral cleft lip.Methods Twenty one patients with unilateral cleft lip grade Ⅱ and unilateral cleft lip grade Ⅲ were observed in the experiment.The related anatomical marks were measured before and after the cheiloplasty respectively.Results ①There was a statistically significant difference in the upper lip height and the height of nose of the operated side of cleft lip grade Ⅲ and cleft lip grade Ⅱ with Tennison technique( P <0.05).②By Millard lip repair,both the height of nose and the nostril floor width of the operated side of cleft lip grade Ⅲ and that of cleft lip were improved significantly ( P <0.05).But the upper lip height of the operated side of cleft lip grade Ⅲ was shorter than that of the control side.Conclusion Tennison lip repair operation should be used to cure unilateral cleft lip grade Ⅲ with little or serious nasal malformation; on the other hand,Millard lip repair technique is better than Tennison to remedy the abnormalities on nasolabial region of unilateral cleft lip grade Ⅱ.
