^(89)Sr改善晚期肿瘤骨转移患者骨痛的研究

摘　　要：目的探讨89Sr治疗骨转移瘤对晚期肿瘤患者生活质量改善的价值。方法对99m锝 MDP全身骨显像证实有多发性骨转移瘤者 6 1例 ,使用89Sr治疗后 ,观察治疗前后疼痛缓解程度、疼痛发作频率及骨转移瘤显像的变化。结果6 1例中 4 3例疼痛出现不同程度缓解 ,缓解率为 70 .5 % (4 3/ 6 1例 )。 6例 (均为轻中度疼痛病例 )疼痛完全消失 ,消失率 9.8% (6 / 6 1例 ) ;1 3例剧痛患者均出现缓解。治疗前后差异有显著性 (u=4 .6 36 ,P <0 .0 1 )。治疗后 4 5例疼痛发作频率明显降低 (u =3.6 4 2 ,P <0 .0 1 )。无须用易成瘾类镇痛剂 ,生活质量得到改善。 6 1例从显像复查对比中 ,发现 1 8例的病灶有不同程度的缩小和减少。病灶好转率为 2 8.6 %。Objectives To evaluate the clinical significance of 89 Sr therapy in treatment of bone metastasis for improving the life quality of patients with advanced stage tumors.MethodsDisseminated bone metastases were confirmed by 99m Tc MDP systemic bone imaging in sixty one patients. The changes of pain remission, pain frequency and bone images before and after 89 Sr therapy were compared in all cases.ResultsForty three cases showed pain remission (70.5%), 45 cases displayed distinct reduction of pain frequency (73.7%;u= 3.642 , P<0.01), administration of addictive analgesics was not needed, so their life quality got improved . Different degree of shrinking and decrease in lesions were observed on 99m Tc MDP bone imaging of 18 cases ( 28.6 %). Significant difference existed between symptoms and signs before and after 89Sr therapy (u= 4.636 , P<0.01) .Conclusion 89 Sr is an effective remedy for remission of pain caused by bone metastases in advanced stage tumors.
