整合素α_5β_1及CD44v6与卵巢癌生物学行为关系研究

摘　　要：目的 :研究黏附分子整合素α5β1及CD4 4v6在卵巢癌中的表达情况及与临床病理因素的关系。方法 :用免疫组化检测6 5例卵巢癌及 2 0例卵巢良性肿瘤的整合素α5β1及CD4 4v6的表达情况 ,分析其与卵巢癌的组织类型、临床分期、细胞分化程度、手术减瘤情况、淋巴结转移情况、CA12 5、无瘤生存期、3年生存率之间的关系。结果 :整合素α5β1及CD4 4v6在恶性和良性上皮性肿瘤中的阳性表达率差异均有显著性 (P <0 .0 1)。整合素α5β1及CD4 4v6的表达情况均与组织学类型无关 (P >0 .0 5 ) ;整合素α5β1的表达与临床分期无关 (P >0 .0 5 ) ,而与细胞分化程度有关 ,细胞分化愈差 ,整合素α5β1表达愈低 (P <0 .0 0 5 ) ;CD4 4v6的表达情况与临床分期、细胞分化程度有关 ,分期愈晚 ,细胞分化愈差 ,CD4 4v6表达愈高 (P <0 .0 5和P <0 .0 0 5 )。整合素α5β1表达与血CA12 5呈负相关 ,CD4 4v6表达与CA12 5呈正相关 (P <0 .0 0 5 )。整合素α5β1阴性表达者淋巴结转移率高 ,CD4 4v6阳性表达者淋巴结转移率高 (P <0 .0 5 )。整合素α5β1表达情况与减瘤满意程度及无瘤生存期均无明显关系 (P >0 .0 5 ) ,但与 3年生存率有关 (P <0 .0 5 )。CD4 4v6表达情况与减瘤满意程度、无瘤生存期及 3年生存率均有关 (P <0 .0Objective:To study the expression of adhesion molecule integrin α 5β 1 and CD44v6 in epithelial ovarian cancers and to analyze the correlation between integrin α 5β 1,CD44v6 and clinicopathological factors of ovarian cancers.Methods:Immunohistochemistry was used to detect the expression of integrin α 5β 1 and CD44v6 in 20 cases of benign epithelial ovarian tumors and 65 cases of malignant ovarian tumors treated between January 1994 and January 1998.The correlation of integrin α 5β 1,CD44v6 and histological types,clinical stages,cell differentiation,residual tumors after cytoreductive surgery,lymph nodes metastasis,CA125,disease free period and 3 year survival rate of epithelial ovarian cancer were analyzed.Results:The difference of positive rate of integrin α 5β 1 and CD44v6 in the malignant and benign epithelial ovarian cancer was significant( P <0.01).Their expressions were not related to histological types( P >0.05).The expression of integrin α 5β 1 was not related to clinical stages( P >0.05),but it had association with differentiation.The poorer the cell was differentiate,the lower the expression would be( P < 0.05 ).The expression of CD44v6 was related to clinical stages and cell differentiation,the later stage it was,the poorer the differentiation,so was the expression higher( P <0.05 and P <0.005).The relationship between positive rate of integrin α 5β 1 and CA125 was negative,while CD44v6 was positive( P <0.05).The metastasis rate of lymph nodes was higher in patients with negative integrin α 5β 1 and patients with positive CD44v6( P < 0.05 ).The expression of integrin α 5β 1 was not related to residue tumors or disease free period( P >0.05),but it had association with the 3 year survival rate( P <0.05);The expression of CD44v6 was related to residue tumors,disease free period and 3 year survival rate( P <0.05～ P <0.005).Conclusions:Expression of integrin α 5β 1 and CD44v6 in epithelial ovarian cancer is significantly associated
