标记免疫分析肿瘤标记物监护TPS定向^(125)I粒子植入治疗肿瘤的研究

摘  要:目的 :利用放射免疫分析血清中多项肿瘤标记物以评价计算机三维治疗计划系统 (TPS)指导下放射性核素1 2 5I粒子手术中植入肿瘤内的疗效。方法 :手术中植入1 2 5I粒子以治疗 78例肿瘤患者 (脑 8人 ,肺 15人 ,肝 12人 ,前列腺 7人 ,直肠 10人 ,胰腺 8人 ,胆管 6人 ,乳房 10人及恶性畸胎瘤 2人 )。脏器的吸收剂量 90~12 0Gy ,放射性活性 370~ 92 5MBq。随访 6~ 12个月 ,根据病种 ,以放射免疫法检测相应有关的肿瘤标记物血清水平 (包括CYFRA2 1- 1,AFP ,CEA ,PSA ,CA199,CA15 3)。结果 :近期疗效甚为满意 ,9 0 %肿瘤完全消失 ,4 1 0 %肿块明显缩小 (>1 2 ) ,5 0 %肿块极度缩小 (>1 3) ,相应的病人血清中肿瘤标记物水平明显降低。结论 :肿瘤病人血清中肿瘤标记物水平的测定是评估放射核粒子植入法疗效的灵敏而可靠的手段。Objective To assess the therapeutic effect of TPS directed implantation of radioseeds 125 I into tumor tissue with Radioimmunoassay of tumor markers. Methods Radio-active seeds 125 I was implanted intra-operatively into 78 patients with malignanay (brain 8, lang 15, liver 12, prostate 7, rectun 10, pancreas 8, bileduct 6, breast 10 and malignant teratoma 2). The organ absorption dose was 90~120 Gy and radio activity was 370~925 MBq. Patients were followed up for 6~12 months and Radioimmunoassays of vrions tumor markers serum levels (CYFRA21-1, AFP, CEA, PSA, CA119, CA153) were performed as suitable for the specific disease entity. Results Reduction of tumor mass was very remarkable (9.0% complete regression, 41.0% regression>1/2 and 50% regression >1/3). Correspondingly, the tested serum tumer marker levels were all significantly lowered. Conclusion For assessing the therapeutic effect of radioactive seeds 125 I implantation, determination of tumor marker levels could be a very sensitive and reliable means.
