周围型肺癌CT征象与PCNA和nm23表达相关性研究

摘　　要：目的 探讨周围型肺癌CT征象与增殖细胞核抗原(PCNA)和nm23表达的关系。资料与方法 对33例经病理证实的周围型肺癌采用SP免疫组化法,检测肿瘤标本中的PCNA和nm23的表达,并分析其与术前CT征象间的关系。结果 PCNA表达与肺癌的组织学分型无关(P>0.05),而nm23表达与肺癌的组织学分型有关,鳞癌nm23表达较高(P<0.01)。PCNA表达与瘤体大小、深分叶征、棘状突起、空泡征、胸膜凹陷征、纵隔淋巴结转移等CT征象有关(P<0.05或P<0.01),与毛刺征无关(P>0.05);nm23表达与上述CT征象均无关联(P均>0.05)。结论 周围型肺癌CT征象中瘤体直径>3cm,或出现深分叶征、棘状突起、空泡征、胸膜凹陷征和纵隔淋巴结转移者,具有较高的恶性度。Objective To investigate the relation between CT signs and the expression of proliferating cell nuclear antigen (PCNA) and the expression of nm23 in peripheral lung cancer. Materials and Methods The expression of PCNA and nm23 was estimated with SP immunohistochemical technique in 33 cases with pathologically proved peripheral lung cancer. The relation between preoperative CT signs and the expression of PCNA and nm23 was retrospectively analyzed. Results The expression of PCNA carried no relation with the type of lung cancer (P > 0.05), while the expression of nm23 showed significant relation with the type of lung cancer. The expression of nm23 in squamous carcinoma was significantly higher than that in other types ( P < 0.01) . The expression of PCNA was related to the lesion' s size and to the presence of deep lobulation, spinous protuberance, vacuole sign, pleural indentation sign and metastasis of mediastinal lymph node ( P < 0.05 or P < 0.01), though it showed no relation to spiculation sign ( P > 0.05). The expression of nm23 was not related to all the above CT signs ( P > 0.05) . Conclusion High malignancy is suggested for peripheral lung cancer when the lesion is over 3 cm, or when the CT signs, such as deep lobulation , spinous protuberance, vacuole sign, pleural indentation sign and metastasis of mediastinal lymph node, are present.
