幽门螺杆菌感染和武威地区胃癌组织中核增殖抗原表达的关系

摘　　要：目的　探讨Hp感染与武威高发区胃癌发生的关系及可能的致癌机制。 方法　Warthin -Starry银染色法判断所选组织Hp的感染状况 ;免疫组化S -P法检测所选标本胃粘膜中核增殖抗原的表达情况。结果　胃癌组Hp感染率为 83 .3 3 %,癌前病变组Hp感染率为 92 .1 1 %,慢性浅表性胃炎组Hp感染率为 73 .0 8%,其中癌前病变组显著高于慢性浅表性胃炎组 (P <0 .0 5 )。Hp阳性组核增殖抗原的表达率明显高于Hp阴性组 (P <0 .0 5 )。结论　Hp感染和武威地区胃癌和癌前病变的发生有关 ;Hp感染可促进胃上皮细胞过度增殖 ,这种过度增殖可能是Hp致胃癌的机制之一。Objective To investigate the relationship between the Helicobacter pylori (Hp) infection and the gastric carcinogenesis in Wuwei area, where a high incidence of gastric carcinoma(GC) exists. To explore the possible gastric carcinogenesis mechanism of Hp.Methods The status of Hp infection was detected by Warthin-Starry Sliver staining. The S-P immunohistochemical method was used in detecting the proliferating cell nuclear antigen(PCNA)expression.Results The numbers of Hp infected rate in GC group was 83.33%, in gastric precancerous lesions (GPL)group was 92.11% and in chronic superficial gastritis (CSG)group was 73.08%, respectively. The positive rate of Hp infection was significantly higher in the GPL group than that of CSG (P<0.05). The positive expression rate of the PCNA in GC was 92.68%, which was significantly higher than that in GPL 75.68% (P<0.05), and in CSG 57.69% (P<0.01). The positive expression rate of PCNA was significantly higher in the Hp positive group (82.76%) than that in the negative group (52.94%). Conclusions Hp infection plays an important role in the pathogenesis of CSG, GPL and GC in Wuwei area. The Hp infection could promote the epithelia cell proliferation, thus, leading to a possible increase in risk of GC.
