摘 要:应用 Ag-NOR_s银染技术及图象分析手段,对乳腺癌、乳腺腺病和正常乳腺组织进行了 Ag-NOR_s 检测及7项核形态参数测定.正常乳腺与Ⅰ级腺病间 Ag-NOR_s 数目的差异无显著性(P>0.05),且 Ag-NOR_s 大小、形态亦无明显差别.Ⅱ级腺病的 Ag-NOR_s 数目大于Ⅰ级腺病(P<0.01),而小于乳癌(P<0.01),但与乳癌 Ag-NOR_s 数目有重叠,且 Ag-NOR_s大小、形态亦与乳癌有相似之处.随着 Ag-NOR_s 数目增加,乳癌组织学级别亦增高.故对乳癌 Ag-NOR_s 数目的检测,有助于预后的估计.7项核形态参数除形态因子外,其余参数均显示正常乳球与Ⅰ级腺病间的差异无显著性(P>0.05),Ⅰ、Ⅱ级腺病及乳癌间的差异均存在高度显著性(P<0.01),与 Ag-NOR_s 数目基本相对应.对于 Ag-NOR_s 数目增多,细胞核面积、平均直径、周长增大及 DNA 含量增加的腺病,应警惕其癌变可能.Nucleolar organizer regions associated proteins(Ag-NOR_s)were identified by means of an argyrophilic technique and 7 morphonuclear parameters were measured by means of an automatic image analysis system(Quantiment 970)in 6 normal breast tissues, 49 adenoses and 41 breast carcinomas.The mean number of Ag-NOR_s‘dots’per nucle- ar for adenosis Ⅱ group(with atypical hyperplasia)was greater than that observed in adenosis Ⅰ group(without atypical hyperplasia).The number of Ag-NOR_s was much highger in the breast carcinoma group.There was an overlap of the nuclear Ag-NOR_s count between the adenosis Ⅱ group and the carcinoma group.The size,shape and distri- bution of Ag-NOR_s‘dots’in the adenosis Ⅱ group were also similar to that in the carcinoma group.These findings indicated the close relation between adenosis Ⅱ and carcinoma.The histological grading of breast carcinoma increases with the increasinig of the numbers of Ag-NOR_s.Thus,Ag-NORs counting is useful for prognostic evaluation. Apart from the shape factor,the other 6 morphonuclear parameters(Area,mean diameter, perimeter,DNA ploid,mean optical density,integrated optical density)were significantly didfferent among adenosis Ⅰ,Ⅱ and breast carcinoma.The above results indicated that the Ag-NOR_s counting and morphonuciear measuring are significant for discriminating adenosis Ⅰ from adenosis Ⅱ and for studying the relation between adenosis Ⅱ and breast carcinoma.If we find the numbers of Ag-NOR_s,nuclear area,mean diameter,perimeter and DNA content increased in some adenosis,it should be aware of the possibility of ma- lignant transformation.
