摘 要:目的 :了解HBV感染者血清HBeAg阴转后HBVDNA含量与C基因启动子 (basalcorepromoter,BCP)和C基因 (pre-core/core ,preC/C)变异的关系。方法 :采用PCR微板核酸杂交方法检测 342例血清HBeAg阴性HBV感染者血清HBVDNA含量及BCP与preC/C基因变异。结果 :在HBsAg/HbcAb/HbeAb和HBsAg/HbcAb感染模式中检测到HBVDNA阳性率为 2 2 8% (36 / 15 8)及 31 6 % (31/ 98)。两者无显著差异 (P >0 0 5 ) ;两种模式中HBVDNA阳性血清的BCP变异检出率分别为 ;6 3 9% (2 3/ 36 )、 5 1 6 % (16 / 31) ;preC/C变异检出率 77 8% (2 8/36 )、 5 8 1% (18/ 31) ;HBVDNA血清浓度平均为 4 9 2± 36 7pg/ml、 5 1 7± 31 5pg/ml,均无显著差异 (P >0 0 5 )。两种基因变异间无相关关系 (P >0 0 5 )。但BCP变异组HBVDNA (6 1 8± 34 1pg/ml)显著高于非变异组 (34 4±2 9 7pg/ml) ;preC/C变异组HBVDNA (5 5 7± 31 5pg/ml)显著高于非变异组 (38 7± 2 4 1pg/ml)。结论 :BCP基因变异与preC/C变异是随机存在于HBeAg转换中 。Objective:To evaluate the clinical relationship between HBeAg conversion,HBV DNA concentration and HBV gene mutation in serum of patients with HBV infection.Method:HBV DNA extracted from 342 HBV infected patients with HBeAg negative were assayed HBV DNA concentration.Basal core promoter(BCP)and pre-C/C gene mutation using sandwich hybridization-ELISA assay on microplate.Results:HBV DNA could be detected in immunological model of HBsAg/HBcAb and HBsAg/HBcAb/HBeAb 22 8%(36/158)?31 6%(31/98)respectively( P >0 05)?Serum HBV DNA concentration.BCP and pre-C/C gene mutation ratio among serum HBV DNA positive subjects with HBsAg/HbcAb were 51 7±31 5pg/ml,51 6%(16/31),58 1%(18/31),and these with HBsAg/HBeAb/HBeAb were 49 2±36 7pg/ml,63 9%(23/36),77 8%(28/36)respectively,all of which were no significance( P >0 05.BCP and preC/C gene mutation were no correlation(X 2=1 2, P >0 0)?HBV DNA level of subjects with BCP mutation and pre-C/C gene mutation were 61 8±34 1pg/ml,55 7±31 5pg/ml respectively,these of subjects with BCP non-mutation and pre-C/Cgene non-mutation were 34 4±29 7pg/ml,38 7±24 1pg/ml,which were significantly different( P <0 05).Conclusion:HBV BCP gene mutation and HBV preC/C gene mutation are in the course of HBeAg conversion randomly and the level of HBV DNA in patients with BCP and preC/C gene mutation is higer than that of patients without the same gene mutation.
