HE4检测对子宫内膜癌的诊断价值

摘　　要：目的研究血清人附睾分泌蛋白4(HE4)对子宫内膜癌的诊断价值。方法术前经子宫内膜活检证实为子宫内膜癌的105例患者为研究组,同期107例良性子宫疾病患者为对照组,留取血清样本检测HE4和CA125水平。CA125的正常值界定为<35 k U/L。HE4设定两个界值点分别为:70 pmol/L和150 pmol/L。结果 CA125对子宫内膜癌的诊断灵敏度为20.0%,而HE4以70 pmol/L和150 pmol/L作为界值点的敏感度分别为58.1%和35.2%。HE4特异性100%(阳性预测值为100%,两个界值点的阴性预测值分别为74.3%和61.2%),而CA125特异性为63.6%(阳性预测值为35.0%,阴性预测值为44.7%)。结论 HE4可作为术前评估子宫内膜癌危险分层的新指标。HE4设定70pmol/L为界值点时敏感性和特异性最佳。Objective To explore the diagnostic value of serum human epididymis secretory protein E4( HE4) in endometrial carcinoma. Methods HE4 and carbohydrate antigen 125( CA125) in serum specimens from 105 cases of endometrial carcinoma( study group) and 107 cases of uterus benign disease( control group) were detected. The normal value of CA125 was considered less than 35 KU / L. The 2 HE4 cutoff were 70 pmol / L and less than 150 pmol / L. Results The sensitivity of CA125 in detecting endometrial cancer was 20. 0%,whereas the sensitivity of HE4 were 58. 1 and 35. 2 % in 70 pmol / L and 150 pmol / L cutoff,respectively. Thus the specificity of HE4 was 100 %( positive predictive value = 100 %,negative predictive value = 74. 3 and61. 2 % in the 2 HE4 cutoff,respectively),whereas the specificity of CA125 was 63. 6 %( positive predictive value = 35. 0 %,negative predictive value = 44. 7 %) in the detection of endometrial cancer. Conclusion HE4 may be a new indicator in preoperative evaluation of endometrial cancer patients. HE4 has the best sensitivity and specificity at cutoff of 70 pmol / L.
