微创介入疗法对肝癌患者T细胞免疫功能的影响分析

摘　　要：目的比较肝动脉栓塞化疗(TACE)和射频消融(RFA)治疗肝癌患者的T细胞免疫功能变化。方法将90例肝癌患者随机分为TACE组和RFA组,检测患者血清中的AST和ALT水平变化,比较患者的CD3+,CD4+,CD8+和CD4+/CD8+水平;比较不同直径的肿瘤患者治疗前后的CD4+/CD8+水平。结果患者治疗过程中AST水平均无明显变化(P>0.05),2组患者不同时间的AST水平无统计学差异(P>0.05)。患者手术后第1天的ALT水平较治疗前明显增加(P<0.05),术后第7天与治疗前无明显差异(P>0.05)。2组患者术后第1天的ALT水平具有统计学差异(P<0.05)。不同大小的肿瘤患者不同时间的CD4+/CD8+水平无统计学差异(P>0.05)。2组患者术后第7天的CD3+,CD4+和CD4+/CD8+具有统计学差异(P<0.05),而CD8+无统计学差异(P>0.05)。结论 RFA对肝癌患者的T细胞免疫功能有促进作用,而TACE则会抑制患者的T免疫功能。Objective To compare the T-cell immune function change of liver cancer patients treated with transcatheter arterial chemoembolization(TACE) and radiofrequency ablation(RFA).Methods 90 cases of liver cancer were randomly divided into TACE group and RFA group,The change of serum AST and ALT levels were detected,and levels of CD3 +,CD4 +,CD8 + and CD4 +/CD8 + were compared.The levels of CD4 +/CD8 + in tumors of different diameters before and after treatment were also compared.Results AST level in patient had no significant changes(P > 0.05),levels of AST at different periods of the 2 groups showed no significant difference(P > 0.05).ALT level of patients 1 d after operation significantly increased than before treatment(P < 0.05),but ALT level of patients 7 d after operation and before treatment(P > 0.05) had no significant difference; ALT level 1 d after operation in the 2 groups had statistical differences(P < 0.05).CD4 +/CD8 + levels in tumors of different sizes had no significant difference at different periods(P > 0.05).CD3 +,CD4 + and CD4 +/CD8 + 7 d after operation in the 2 groups showed significant difference(P < 0.05),but CD8 + levels had no significant difference(P > 0.05).Conclusion RFA has positive effect on T-cell immune function in patients with hepatocellular carcinoma yet TACE can inhibit T-cell immune function.
