苦参黄连汤加地塞米松保留灌肠联合柳氮磺胺吡啶治疗溃疡性结直肠炎随机平行对照研究

摘　　要：[目的]观察苦参黄连汤加地塞米松保留灌肠联合柳氮磺胺吡啶治疗溃疡性结直肠炎疗效。[方法]使用随机平行对照方法,将80例门诊患者按随机数字表分为两组。对照组40例柳氮磺胺吡啶肠溶片,1g/片,2.0～3.0g/次,3～4次/d。治疗组40例苦参黄连汤(苦参、黄连、黄柏、赤芍、大黄、乳香、白头翁、地榆、白友、血竭、枯矾、白芷、玄胡、三七各30g)。浓煎100mL,1剂/2d;水温冷却到20～30℃时,加地塞米松,10mg/片,每晚排便后保持灌肠,1剂/d。西药治疗同对照组。连续治疗7d为1疗程。观测临床症状、大便性状、结肠黏膜、复发率、不良反应。连续治疗4疗程,判定疗效。随访12个月。[结果]治疗组治愈32例,显效24例,有效17例,无效7例,总有效率91.25%。对照组治愈26例,显效22例,有效16例,无效16例,总有效率80.00%。治疗组疗效优于对照组(P<0.05)。治疗组复发比率为21.11%,对照组复发比率为59.4%,复发率治疗组优于对照组(P<0.05)。[结论]苦参黄连汤加地塞米松保留灌肠联合柳氮磺胺吡啶治疗溃疡性结直肠炎疗效显著,复发率低,副作用少,值得推广。[Objective]Observe Kushen Huanglian Decoction and Dexamethasone combination sulfasalazine enema treatment for ulcerative colitis Efficacy.[Methods]Using randomized controlled method,80 outpatients were randomly divided into two groups.Control group,40 cases of sulfasalazine enteric-coated tablets,1g / tablets,2.0 ～ 3.0g / time,3 to 4 times / d.Berberine treatment group,40 cases of Sophora soup(Kushen,Huanglian,Huangbai,Chishao,Dahuang,Ruxiang,Baitouweng,Diyu,Baiyou,Xuejie,Kufan,Baizhi,Xuanhu,Sanqi each 30g).Thick fried 100mL,1 does / 2d;temperature cooled to 20 ～ 30℃,the plus dexamethasone,10mg / tablets,night after defecation remain enema,a / d.Western medicine with the control group.7d is a continuous course of treatment.Observation of clinical symptoms,stool,colonic mucosa,the recurrence rate of adverse reactions.Four cycles of continuous treatment,to determine efficacy.Followup of 12 months.[Results]treatment group were cured 32 cases,effective in 24 cases,effective in 17 cases,7 cases,the total efficiency of 91.25%.Control group,26 cases were cured,22 cases markedly effective in 16 cases,ineffective in 16 cases,total effective rate 80.00%.Treatment group than the control group(P <0.05).Treatment group,the recurrence rate was 21.11% in the control group,the recurrence rate was 59.4% and the recurrence rate in treatment group than the control group(P <0.05).[Conclusion]Kushen Huanglian Decoction and Dexamethasone combination sulfasalazine enema treatment for ulcerative colitis effect,recurrence rate,less side effects,worthy of promotion.
