摘 要:目的 :研究血管内皮生长因子 (VEGF)在星形细胞胶质瘤生长过程中起的作用 ,探讨它们之间的相关性。方法 :实验采用免疫组化S -P法对不同恶性程度星形细胞瘤及正常脑组织中VEGF和第八因子相关抗原 (FⅧRAg)进行检测 ,同时行电镜观察 ,找出星形细胞瘤恶性程度与VEGF分泌程度的相关性。结果 :(1 )不同恶性程度肿瘤之间术前均有不同程度的脑水肿 ,肿瘤级别高脑水肿程度重 (F =1 5 95 ,P <0 0 5)。(2 )正常脑组织、血管及神经胶质细胞均无VEGF表达 ,随肿瘤恶性程度增高VEGF表达逐渐增强 ,Ⅲ、Ⅳ级最为显著 (F =1 2 0 8,P <0 0 5)。 (3)不同恶性程度肿瘤微血管密度均有差异 (F =1 6 65 ,P <0 0 5)。 (4)电镜下见星形细胞瘤Ⅰ、Ⅱ级胶质细胞核异型性不大 ,血管内皮细胞均匀 ;Ⅲ级以上肿瘤血管内皮细胞间隙扩张、形状不规则、核大 ,有的变性坏死、线粒体肿胀空泡化、基膜肿胀疏松、部分中断缺损 ,血管壁厚薄不均 ,血管周围间质水肿明显。结论 :VEGF通过刺激肿瘤血管形成、增加血管通透性来促进肿瘤的生长、转移 ,引起肿瘤周围脑水肿 ;人脑星形细胞瘤的恶性程度与VEGF呈正相关。Objective:To study the role of VEGF during the course of development of astrocytoma and investigate the correlation between them.Methods:Experiment was taken by the method of“S-P”Immunostaining to VEGF of the different malignant degree tumor and the normal brain tissue and FⅧRAg.At the same time,under the electron microscope to find the correlative of VEGF and the malignant degree in human astrocytoma.Results:There are different degree cerebal edema in the tumors of different malignant degree,the more malignant,the more serious edema(F=15.95,P<0.05).There is no expression in the normal brain tissue,blood vessel and the nervous gliocyte,with the development of tumor malignant degree,expression of VEGF is progressively increasing.Grade Ⅲ and grade Ⅳ are the most significant(F=12.08,P<0.05).Microvasculature density has significant difference between the different malignant degree tumors(F=16.65,P<0.05).Observation under electron microscope:There is no significant cell change of structure in astrocytoma of grade Ⅰ and gliocyte neclie of grade Ⅱ,but we can observe obviously change by electron microscopic at the grade Ⅲ,such as vascular endothelial cell,mitochondria,basement membrane defect and vascular wall.Conclusions:VEGF promot the growth and transfer of tumor mainly through stimulating angiogenesis and increasing vescular permeability,and lead to tumor surroundings edema.The malignant degree of brain astrocytoma is positive correlation with VEGF.
