紫杉醇联合顺铂、醛氢叶酸和氟尿嘧啶治疗晚期胃癌近期疗效观察

摘　　要：①目的　评价紫杉醇联合顺铂、醛氢叶酸和氟尿嘧啶治疗晚期胃癌疗效及安全性。②方法　 5 2例胃癌病人 ,采用紫杉醇 175mg/m2 ,静脉滴注 ,第 1、8天 ;顺铂 4 0mg/m2 ,静脉滴注 ,第 2～ 4天 ;醛氢叶酸 2 0 0mg/m2 ,静脉滴注 ,第 2～ 4天 ;氟尿嘧啶 2 5 0mg/m2 ,静脉推注 ,第 2天 ,然后 15 0 0mg/m2 ,72h持续静脉滴注。 2 1d为 1个周期 ,治疗 2个周期后评定疗效。③结果　 5 2例均可评价疗效 ,其中完全缓解 3例 ,部分缓解 2 4例 ,总有效率为 5 1.9%。不良反应主要表现为白细胞减少和恶心、呕吐。④结论　紫杉醇联合顺铂、醛氢叶酸和氟尿嘧啶是治疗晚期胃癌较好的化疗方案。Objective\ To evaluate the efficacy and toxicity of combination use of paclitaxel (TAX) with cis platin (CDDP) , leucovorin (LV) and fluorouracil (Fu) for advanced gastric cancer. \ Methods\ Fifty two patients with advanced gastric cancer were treated. The treatment schedule was composed of TAX 175 mg/m 2, iv drip, day 1 and day 8; CDDP 40 mg/m 2, iv drip, day 2-4; LV 200 mg/m 2, iv drip, day 2-4; Fu 250 mg/m 2, iv, day 2, and 1 500 mg/m 2 infusion for 72 h. A treatment cycle lasted 21 days. The efficacy was evaluated after two cycles of the chemotherapy.\ Results\ Complete response in three cases and partial response in 24 cases were recorded, with the overall response rate being 51.9%. The main reactions of toxicity were neutropenia and nausea as well as vomiting.\ Conclusion\ The combination chemotherapy is a high response regimen for advanced gastric cancer with mild toxicity.
