改良小梁切除术治疗青光眼的远期疗效

摘　　要：目的对改良小梁切除术治疗各类青光眼的远期疗效进行临床研究,并评估此术式的远期效果。方法回顾性分析1980-1990运用改良小梁切除术治疗各类青光眼118例(156眼),其中闭角型青光眼128眼,开角型青光眼11眼,继发性青光眼9眼,先天性青光眼8眼,并自拟定Ⅰ-Ⅳ级疗效标准。结果经7～10a的随访观察,术后1、2、3、5、10a手术成功率分别为92.3％、90.1％、87.3％、80.4％、78.3％。远期并发症:并发白内障10眼(6.4％),瘢痕粘连眼压升高20眼(12.8％)。结论长期随访观察及临床研究显示改良小梁切除术设计合理、操作简便安全可靠,并发症少,疗效满意,易于推行。Aim To investigate the long-term effects of refined trabeculectomy for glaucoma. Methods By random sampling, 118 cases (156 eyes) of glaucoma treated with refined trabeculectomy between 1980 and 1990 were selected and studied, including 128 eyes of close-angle glaucoma, 11 eyes of open-angle glaucoma, 9 eyes of secondary glaucoma and 8 eyes of congenital glaucoma. In order to determine the long-term effect, a 4-grade standard was devised, from Ⅰ to Ⅳ. The follow up ranged from 7 to 10 years. Results The success rate at 1 year, 2 years, 3 years, 5 years after the operation was respectively 92.3%, 90.1%, 87.3%, 80.4% and 78.3%. Long-term complications included increased intraocular pressure related to cicatricial adhesion in 20 eyes (12.8%) and cataract in 10 eyes (6.4%). Conclusion Long-term observations have confirmed the effect of refined trabeculectomy.
