VEGF和sCD44v6在恶性腹水中的诊断及意义探讨

摘　　要：目的　探讨VEGF、sCD4 4v6对恶性腹水的诊断价值及其在恶性腹水形成中的作用。方法　收集各种类型腹水及腹腔液 ,采用ELISA法检测VEGF、sCD4 4v6水平。结果　癌性腹水组VEGF、sCD4 4v6水平明显高于肝硬化腹水组、结核性腹水组及良、恶性疾病不伴腹水患者腹腔液组 (P均 <0 .0 1) ,而后 4组间比较VEGF及sCD4 4v6水平则均无明显差别 (P >0 .0 5 )。不同恶性肿瘤腹水中卵巢癌组VEGF水平高于胃癌组和结肠癌组 (P均 <0 .0 1) ;胃癌组VEGF水平高于结肠癌组 ;卵巢癌组sCD4 4v6水平高于胃癌组和结肠癌组 ,但差异均无显著性 (P >0 .0 5 )。VEGF诊断恶性腹水的敏感性为91.3% ,特异性为 88.9% ;sCD4 4v6诊断恶性腹水的敏感性和特异性则为 73.9%和 91.7%。结论　VEGF、sCD4 4v6对良、恶性腹水的鉴别诊断有重要价值 ,它们可能在恶性腹水的形成中起重要作用。Objective To explore the diagnostic value of vascular endothelial growth factor(VEGF) and sCD44v6 in malignant ascites and determine their significance in malignant ascites formation. Methods To detect VEGF and sCD44v6 levels in various kinds of ascites and fluids of abdominal cavity by enzyme-linked immunosorbent assay. Results VEGF and sCD44v6 levels in malignant ascites were significantly higher than those of cirrhosis, tuberculous is peritonitisand and of fluids of abdominal cavity of patients with benign and malignant disease but without ascites ( P < 0.01 ). However, there were no significant difference of VEGF and sCD44v6 levels among the latter 4 groups ( P > 0.05 ). VEGF levels in malignant ascites from patients with ovarian cancers were higher than those with gastric and colon cancers ( P < 0.01 ), while sCD44v6 levels in malignant ascites from patients with ovarian、gastric and colon cancers were no significant difference ( P > 0.05 ). The sensitivity and specificity of this assay to diagnose malignant ascites were 91.3% 、 88.9% and 73.9% 、 91.7% respectively. Conclusion The detection of VEGF and sCD44v6 levels is useful in differential diagnosis of benign and malignant ascites. They may play an important role in malignant ascites formation.
