p16、p53和c-erbB-2蛋白在胃癌的表达及临床意义

摘　　要：目的　研究抑癌基因 p16、p5 3和癌基因 c- erb B- 2在胃癌和癌前病变中的表达及其与胃癌临床病理学特征的关系 ,进一步探讨胃癌发生发展的机制。　方法　运用免疫组织化学方法检测 p16、p5 3和 c- erb B- 2蛋白在胃癌和癌前病变中的表达。　结果　 p16蛋白在正常胃粘膜、肠上皮化生、异型增生和胃癌中表达呈下降趋势 ,异型增生和胃癌中 p16蛋白表达显著低于正常胃粘膜 (P<0 .0 5 ) ,p16蛋白表达降低与胃癌分化程度、浸润深度及有无淋巴结转移均无明显相关性 ;p5 3蛋白在胃癌中表达显著高于异型增生 (P<0 .0 5 ) ,并且与胃癌分化程度和淋巴结转移关系密切 (P<0 .0 5 ) ;c- erb B- 2蛋白阳性表达与胃癌浸润深度以及淋巴结转移关系密切 (P<0 .0 5 )。　结论　p16、p5 3和 c- erb B- 2在胃癌的发生发展过程中起着协同促进的作用。检测 p5 3和 c- erb B-Objective\ To investigate the expression of p16, p53 and c\|erbB\|2 gene in gastric carcinoma and precancerous lesion and their correlation with the clinicopathological characteristics of gastric carcinoma.\ Methods\ The expression of p16, p53, c\|erbB\|2 and proliferating cell nuclear antigen(PCNA) monoclone antibody were detected by immunohistochemistry in 69 cases of gastric carcinoma, 54 precancerous lesions and 15 normal gastric mucosae.\ \{Results\}\ The positive rate of p16 gene in gastric carcinoma and precancerous lesions was significantly lower than that in normal mucosa(P<0 05).\ No correlation between the expression of p16 gene and grade of differentiation, infiltration degree and lymph node metastasis was observed.\ The positive rate of p53 gene in gastric carcinoma was significantly higher than that in precancerous lesions(P<0 05).\ There was significant correlation between p53 and grade of differentiation and lymph node metastasis(P<0 05).\ C\|erbB\|2 positive expression was correlated to infiltration degree and lymph node metastasis(P<0 05).\ The abnormal expressions of p16, p53 and c\|erbB\|2 were correlated with PCNA LI(PCNA labeled index, P<0 01).\ \{Conclusion\}\ The abnormal expressions of p16, p53 and c\|erbB\|2 play an important role in the course and development of gastric carcinoma.
