老龄食管癌手术前后动脉血气分析及治疗要点

摘　　要：目的探索老龄食管癌手术治疗诊疗规律。方法分析46例高龄食管癌贲门癌患者住院临床资料。结果食管癌好发于老年人,男性多于女性,下段食管多于中上段,手术后动脉血气分析均明显下降。结论外科手术切除食管癌能使病人进食梗阻得到缓解,并能提高生活质量延长存活期,术后PaO2及SaO2均明显下降,电解质无明显变化。弓下吻合动脉血气分析,氧分压、氧饱和度下降明显低于弓上吻合。Objective Oesophageal cancer is currently one of the common human malignant tumour s.Methods 46case of elderly patients with esophageal and cardia cancer were analysed.Results Gravely threatening the life and hea lth of the population.People above 60years of age are predisposed to this malig nancy.There are more cases in males t han in females,and the lesion is more often seen in th e upper and middle segment and the cardia.The pre -and post -operative determinations of blood gas electrolytes did not show much difference.While post -opera tive blood gas analyses(with oxygen given through nasal tube at 3L /min,or through facial mask at 5L /min)gave significantly lower PaO 2 and SaO 2 values than the pre -operative levels.This fact deserved serious a ttention.Conclusions The pre -and post -operative PaCO 2 and pH values were corre-spondingly different,but the range s were small.In arterial gas analysis,PaO 2 and SaO 2 were markedly lower in intra -arch anastomosis than in supra -arch anastomosis.[
