世界卫生组织生活质量问卷在中国应用的信度及效度研究

摘　　要：目的　评估世界卫生组织生活质量问卷 (WHOQOL 10 0 )在中国慢性疾病患者及其照料者中应用的信度和效度。方法　应用WHOQOL 10 0 (共 2 5个因子 )对 4 6 0例慢性疾病患者和 4 18名家庭照料者的生活质量进行了评定 ,以生活质量综合问卷 (GQOLI)作为评价WHOQOL 10 0在中国文化背景下的校标效度标准。结果　 (1)内部一致性 :WHOQOL 10 0条目的α系数为 0 76～ 0 90 ;(2 )效标效度 :GQOLI的躯体、心理、社会功能、物质条件 4个维度与WHOQOL 10 0相应的躯体、心理、社会交往、生活环境 4个维度均有较高的相关性 (Pearson相关系数分别为 0 77,0 82 ,0 72 ,0 73) ,WHOQOL 10 0的总体生活质量与GQOLI的 4个维度均呈中等程度相关 (r =0 33～ 0 5 9)。(3)结构效度 :在 2 5个因子中 ,除疼痛与不适、自尊、体象、医疗依赖及娱乐 5个因子外 ,余各因子均与其所属的维度存在高相关性 (r =0 6 1～ 0 72 ) ;(4)主成分分析 :产生的躯体、心理、社会功能和环境 4个因素 ,可解释 6 1%的方差。结论　WHOQOL 10 0在中国的慢性疾病患者和照料者中应用有较好的信度和效度。Objective The World Health Organization Quality of Life questionnaires (WHOQOL-100) is a multi-dimensionally conceptualized, generic, 100-item instrument for measuring the quality of life. This study was to evaluate the validity and reliability of its use in the chronic patients and their family caregivers in China.Methods Altogether 460 chronic patients and 418 family caregivers were evaluated with the WHOQOL-100. The validity of WHOQOL-100 was evaluated with comparison to the General Quality of Life Inventory(GQOLI), which was developed in Chinese culture. Results (1) The WHOQOL-100 had acceptable internal consistency with Cronbach′s α range of 0.76-0.90 across domains. (2) The dimensions of WHOQOL-100 were highly correlated to the corresponding dimensions of GQOLI ( r =0.72-0.82). (3) The twenty-five WHOQOL-100 items, except five items (pain and discomfort, esteem, body image and appearance, dependence on medication or treatment, leisure), were highly correlated to the respective dimensions, which indicated a well structure validity. (4) The principal component analysis produced four factors, i.e., physical, psychological, social and environmental factor, which accounted for 61% of the total variance. Conclusion The WHOQOL-100 is a reliable and valid instrument for assessing the QOL of chronic patients and family caregivers in Chinese culture.
