提高乳腺癌早期诊断水平的研究

摘　　要：目的 :评价育龄妇女乳腺疾病普查、乳腺钼靶摄片及 99m Tc-MIBI核素显像在乳腺癌早期诊断中的价值 ,探索提高乳腺癌早期诊断的方法。方法 :对徐州地区 10万例育龄妇女进行乳腺疾病的普查 ,筛选出的乳腺癌高危人群及门诊就诊的乳腺疾病患者行乳腺钼靶摄片及 99m Tc-MIBI核素显像检查。结果 :普查乳腺癌检出率为 2 8.9/10万。乳腺钼靶摄片诊断乳腺癌的灵敏度为 90 .2 %、特异性为 3 1.4%。99m Tc-MIBI核素显像灵敏度为 82 .5% ,特异性为 90 .2 %。联合应用乳腺钼靶摄片与 99m Tc-MIBI核素显像 ,对乳腺癌诊断的灵敏性为 10 0 % ,特异性为 99.3 %。结论 :育龄妇女乳腺疾病普查 ,结合乳腺钼靶摄片及 99mObjective:To appraise the value of general survey of breast diseases in adult women and application of mammography and 99m Tc-MIBI scintommmography for early diagnosis of breast carcinoma, and to invest egate the methods of diagnosis for breast carcinoma.Methods:One hundred thousands adult women in Xuzhou were examin ed for breast diseases. The technique of mammography and 99m Tc-MIBI sci ntommmography were used to diagnose breast carcinoma for high risk people seleced by general survey and patients who suffered breast disease.Results:The results of general survey of adult women demonstrate d that incidence of breast cancer in Xuzhou is 28.9/1×10 5.The sensitivity and speciticity of diagnosis of mammography were 90.2% and 31.4% respectively. The sensitivity and speciticity of 99m Tc-MIBI scintomammography were 82.5 % and 90.2 % respectively. The sensitivity and speciticity by combined utilizat ion of mammography and 99m Tc-MIBI scintomammography were 100 % and 92.8 %.Conclusion:The method of general survey of breast diseases of ad ult women combined mammography and 99m Tc-MIBI scintomammography can imp rove diagnosis of breast carcinoma.
