摘 要:目的 探讨肝素缓释系统 (heparindrugdeliverysystem ,HEPDDS)抑制后发性白内障的有效性和可行性。方法 对 2 5只 5 0眼新西兰大白兔 ,行超声乳化透明晶状体吸出术。随机分为生理盐水滴眼组、载体空白对照组、5 0 g·L-1肝素滴眼液滴眼组、HEPDDS结膜下植入组和HEPDDS后房内植入组。术后行裂隙灯、组织病理学、电镜检查 ,血、房水肝素浓度检测及后囊湿重的称取。结果 术后 12周发生后囊混浊的眼数分别为10、10、8、7、4眼 (P =0 .0 0 2 ) ,后囊湿重均值分别为 15 7.919、16 0 .0 91、81.114、71.82 7、19 984mg(P =0 0 0 0 )。后 3组房水肝素浓度均值分别为 10 .2 79、13.2 4 6、2 5 .96 4mg·L-1(P =0 .0 0 0 )。HEPDDS后房内植入组后囊细胞增生不活跃 ,未发现眼内毒性反应。术中、术后无眼内出血现象。结论 后房内植入肝素缓释系统 ,可有效抑制后发性白内障的发生 ,是一种安全有效的给药方法。Objective To investigate the safety and efficacy of heparin drug delivery system(HEP DDS) on the experimental animals for inhibition of after cataract. Methods Twenty five New Zealand rabbits(50 eyes) received phacoemulsification, and then were randomly divided into 5 groups: normal saline eye dripping group(group A), posterior chamber implantation of carrier DDS group(group B), 50g·L -1 heparin eye dripping group(group C), subconjunctival implantation of HEP DDS group(group D) and posterior chamber implantation of HEP DDS group(group E). Fifty eyes in the 5 groups were examined by slit lamp microscopy, the blood and aqueous humor heparin concentrations were measured, and the wet posterior capsules were weighed.Results At 12 weeks after operation, the occurrence of posterior capsule opacification was noticed in 10,10,8,7,4 eyes respectively in the 5 groups( P =0.002), the average weight of the wet posterior capsules of the 5 groups was 157.919, 160.091 ,81.114, 71.827 ,19.984mg respectively( P =0.000). The average value of the aqueous humor heparin concentration in group C, D and E was respectively 10.279,13.246,25.964mg·L -1 ( P = 0.000 ). The cell proliferation in the posterior capsules of group E was not active, and there's no obvious toxic reaction in the eyes. There was no intraocular hemorrhage during the follow up period.Conclusion The implantation of HEP DDS into the posterior chamber can effectively prevent the after cataract, and is proved to be a safe and effective medication.
