摘 要:目的 观察海水浸泡大鼠在不同低体温情况下血气和酸碱代谢的改变。方法 将 3组大鼠分别置于 19~ 2 1°C海水中游动 ,于肛温分别降至 35 ,30及 2 5°C时 ,快速自左心室取血检测血气和酸碱 ,最后断头处死动物 ,取肺组织进行 HE染色光镜观察。结果 随体温降低 ,动脉血氧分压 (Pa O2 )、肺泡内氧分压 (PAO2 )、动脉血氧分压与肺泡内氧分压比 (Pa O2 / PAO2 )、血液中氧分压与吸氧浓度之比(PO2 / FIO2 )、氧含量 (CO2 )及阴离子隙 (AG)逐渐上升 ,当大鼠体温降至 2 5°C时 (2 5°C组 ) ,Pa O2 测得值(2 4 .86± 3.6 4 ) k Pa、PAO2 (111.75± 6 .38) k Pa、Pa O2 / PAO2 1.4 4± 0 .2 9、PO2 / FIO2 834.90± 6 6 .4 8、CO2(199.0± 18.6 ) ml/ L、AG值 (2 4 .5 2± 5 .5 3) m mol/ L,与正常对照组比较 ,测得值上升非常显著 (P值均<0 .0 1) ;血浆酸碱度 (p H)、细胞外碱剩余 (BE- ECF)、全血碱剩余 (BEb)、血浆总二氧化碳 (TCO2 )、标准碳酸氢盐 (SB)、及实际碳酸氢盐 (AB)逐渐降低 ,当大鼠肛温降至 2 5°C时 (2 5°C组 ) p H达 7.2 1± 0 .0 5、BEb(- 9.97± 2 .5 1) mmol/ L、BE- ECF(- 12 .33± 2 .6 2 ) m mol/ L、TCO2 (14 .12± 2 .5 0 ) m mol/ L、SB值(15 .6 3± 2 .17) mm ol/ L、AB值 (14 .6 2?Objective To study the relationship between changes of blood-gas as well as acid-base imbalance and body temperature in rats immersed in sea-water of different temperatures.Methods 3 groups of rats were put into 19-21 °C sea-water alternatively to swim,and were taken out of sea-water rapidly when their rectal temperature fell to 35 °C ,30 °C and 25 °C ,respectively.Blood-gas and acid-base were examined with 1 ml arterial blood taken rapidly from left ventricle,and lung tissue was observed by using dye HE dying with HE after the rats were executed.Results With body temperature decreasing,PaO\-2,PAO\-2,PaO\-2/PAO\-2,PO\-2/FIO\-2,C O\-2 and AG went up,and reached to (24.86±3.64) kPa,(111.75±6.38) kPa,(1.44±0.29),(834.9±66.48),(199.0±18.6) ml/L,(24.52±5.53) mmol/L respectively in the 25 °C group;all the items above had remarkable differences with those of the normal control group;pH,BE-B,BE-ECF,T CO\-2 ,SB and AB went down remarkably in all the 3 groups,compared with the normal control group( P <0.01),and reached to 7.21±0.05、(-9.97±2.51) mmol/L,(-12.33±2.62) mmol/L,(14.12±2.50) mmol/L,(15.63±2.17) mmol/L,(14.62±2.74) mmol/L respectively in the 25 °C group;while PaCO\-2 had not remarkable change in all the 3 groups,compared with the normal control group ( P >0.05).Conclusions High PaO\-2 and metabolic acidosis became more obvious with the decreasing temperature in rats immersed in sea water;hypothermia may be the cause of high PaO\-2 and metabolic acidosis.
