下颌骨骨折患者围术期咬合功能变化的研究

摘　　要：目的本文利用T-ScanII计算机咬合分析系统,评价下颌骨骨折患者术前和术后的咬合力变化。方法收集20例下颌骨骨折病例,利用T-ScanII计算机咬合分析系统对患者术前、术后1个月和术后3个月的咬合力进行测定,分别对患者的牙合力不对称指数、总牙合力、牙合力中心点位置进行比较。结果患者术后1个月测得的牙合力不对称指数较术前显著下降(P<0.01),术后3个月进一步下降;总牙合力值在术后3个月较术前显著提高(P<0.05),术后1个月的变化与术前比较无显著性差异;牙合力中心点位置在术后趋于正常。结论切开复位内固定治疗可立即重建下颌骨骨折患者的正常咬合,为咬合功能的恢复创造条件;但牙合力的恢复比较缓慢。双侧牙合力的分布、牙合力中心点位置以及牙合力的大小,是评价下颌骨骨折治疗后咬合功能变化的有效手段。Objective In this s tu dy,an occlusion construe system was used to evaluate the occlusal force of the p atients suffered from mandible fractures and treated with ORIF.Methods20cases wi th mandible fracture were investigated in this study.The T-scan II system was us ed to record and analyze the patients'occlusion in three different periods,befo re operation,one month after operation and three months after operation.Asymmetr y index of occlusal force(AOF),total occlusal force(TOF)and center of occlusal f orce(COF)were compared in statistic way.Results The decrease of AOF in one and t hree months after operation was significant,the COF retuned to normal after oper ation and the TOF did not increase one month after operation until three months after operation.Conclusion With the treatment of open reduction and internal fix ation,the malocclusion was corrected to be normal,which was the prerequisite for the rehabilitation of the occlusion function.The occlusal force was increased s lowly after operation compared with the other two indexes.The AOF,TOF and COF we re effective in evaluating the occlusion function of the fractured mandible peri operatively.
