SU6668对结肠癌术后肠吻合口愈合的影响

摘　　要：目的　研究肿瘤血管生成抑制剂SU6668对结肠癌术后肠吻合口愈合的影响。方法将 3 2条犬行模拟结肠癌根治术 ,术后随机分为 4组 ,每组 8头。手术当天开始 ,分别自腹腔注射 :无药物 (对照组 )、5 氟尿嘧啶 (5 Fu) 3 0mg/kg体重、SU 6668制剂 (65mg/kg体重 ,SU 6668组 )、5 Fu与SU6668联合应用 (5 Fu 3 0mg/kg体重 ,SU 666865mg/kg体重 ,联用组 ) ,每天 1次 ,共用 12d。检测各组吻合口破裂压、羟脯氨酸含量、组织病理学改变和胶原纤维超微结构变化 ,评估SU 6668对肠吻合口愈合的影响。结果　术后第 12天 ,所有动物再次剖腹探查 ,均无吻合口瘘的发生。对照组、5 Fu组、SU6668组、联用组的吻合口破裂压分别为 :(3 3 .2± 4.7)kPa(1kPa =7.5mmHg)、(3 2 .5± 5 .1)kPa、(3 0 .8± 5 .9)kPa和 (3 1.4± 4.6)kPa(P >0 .0 5 ) ;羟脯氨酸含量分别为 :(16.7±2 .5 )mg/g ,(15 .2± 1.8)mg/g ,(14 .9± 2 .7)mg/g和 (15 .6± 2 .3 )mg/g(P >0 .0 5 )。组织病理学检查结果显示各组吻合口肠粘膜上皮移行 ,愈合良好。电镜下 ,各组吻合口组织均见胶原纤维交叉紧密排列 ,其间有大量成纤维细胞。结论　SU6668单独或与腹腔化疗药物联用时 ,均对结肠癌术后肠吻合口愈合无不良影响。作为新的外科辅助治疗 ,结肠癌的抗血?Objective To investigate the effects of tumor angiogenesis inhibitors SU6668 on healing of colonic anastomoses in the dog.Methods A imitative radical enterotomy was performed and a colonic anastomosis was constructed in 32 dogs.All the animals were randomly divided into four groups ( n =8) and subjected to different daily intraperitoneal treatment for 12 days as follows:control group not receiving treatment; 5 Fu group,receiving 5 Fu (30 mg/kg); SU6668 group,receiving SU6668 (65 mg/kg) and combined treatment group (5 Fu 30 mg/kg and SU6668 65 mg/kg).The influence of SU6668 on healing of colonic anastomoses was studied by observing and comparing the anastomosis bursting pressure,hydroxyproline content,histopathologic and ultrastructure changes in different groups.Results At the 12th postoperative day,no anastomotic leakage was found in all animals at the second exploratory laparotomy.The averaged anastomosis bursting pressure in the control group,5 Fu group,SU6668 group and combined treatment group was (33.2± 4.7), (32.5± 5.1), (30.8± 5.9) and (31.4± 4.6) kPa respectively ( P >0.05).The contents of hydroxyproline in each group were (16.7±2.5) ,(15.2±1.8),(14.9±2.7) and (15.6±2.3) mg/g respectively ( P >0.05).The histopathologic observation showed well healing in all colonic anastomoses.Conclusion SU6668 has no adverse effect on the healing and resistance of colonic anastomoses.As a new adjuvant surgical therapy,antiangiogenic colon cancer therapy would be safe and feasible in the futrue clinical practice.
