直肠癌旁移行粘膜Ki-67、p53及C-erbB-2的表达及其临床意义

摘　　要：目的　探讨直肠癌旁移行粘膜 (TM )的生物学性质及其临床意义。方法　应用粘液组织化学、免疫组织化学技术对 3 3例直肠癌手术切除标本及其近远端肠管粘膜 ,同步检测其Ki 67增殖指数、p5 3、C erbB 2蛋白表达率。结果　Ki 67指数在远切端移行粘膜较癌组织低 (P <0 .0 1) ,但高于近切端正常粘膜 (P <0 .0 5 ) ;在癌组织与近切端正常粘膜中相比差异亦有显著性(P <0 .0 1)。p5 3蛋白阳性率在远切端TM中为 2 1.2 1% ,癌组织为 5 7.5 8% ,近切端正常粘膜为O ,远切端TM低于癌组织 (P <0 .0 1) ,但高于近切端正常粘膜 (P <0 .0 5 )。C erbB 2蛋白阳性率在远切端TM中为 18.18% ,癌组织为 60 .60 % ,近切端正常粘膜为O ,远切端TM低于癌组织 (P <0 .0 1) ,但高于近切端正常粘膜 (P <0 .0 5 )。结论　直肠癌旁移行粘膜可能是一种原发性癌前病变 ;Objective To investigate the biopathologic characteristics of transitional mucosa (tm) adjacent to rectal carcinoma and its clinical significance.Methods The materials consisted of primary rectal adenocarcinomas ( n = 33),the upper resection margins ( n =33),the down resection margins ( n = 33).The Ki 67 index and the expression rate of p53,C erbB 2 proteins were studied simultaneously by using mucinhistochemistry and immunohistochemistry.Results The Ki 67 index in TM of down resection margins was lower than in tumor tissue ( P < 0.01),but higher than in normal mucosa of upper resection margins ( P < 0.05).The Ki 67 index in tumor tissue was higher sigificantly than in normal mucosa of upper resection margins ( P < 0.01).The expression rate of p53 protein in TM of down resection magins was 21.21%,in tumor tissue was 57.58%,in normal mucosa of upper resection margins was zero.The expression rate of p53 protein in TM of down tesection magins was lower than in tumor tissue ( P < 0.01),but higher than in normal mucosa of upper resection margins ( P <0.05).The expression rate of C erbB 2 protein in TM of down resection magins was 18.18%,in tumor tissue was 60.60%,in normal mucosa of upper resection margins was zero.The expression rate of C erbB 2 protein in TM of down resection magins was lower than in tumor tissue ( P <0.01),but higher than in normal musoca of upper resection margins ( P <0.05).Conclusion TM adjacent to the rectal carcinoma may be a primary premalignant phenomenon.The length of TM of down resection margins should be acted as one of the considerable measures in the anus preserving operation.
