老年人垂体腺瘤低钠血症的识别和处理(附4例报道)

摘　　要：目的　探讨老年人垂体腺瘤术前发生低钠血症的机理、鉴别诊断及治疗。方法　结合文献对 4例垂体腺瘤于自然病程中发生低钠血症进行分析。结果　 4例均为垂体大腺瘤患者 ,年龄超过 6 5岁 ,其低钠血症为水潴留所致 ,限水治疗可改善低钠血症 ,手术切除肿瘤能迅速纠正该低钠血症。结论　老年人垂体腺瘤发生低钠血症与肿瘤压迫视丘下部有关 ,表现类似抗利尿激素分泌异常综合征 (SIADH)。Objective To study mechanism,differential diagnosis and treatments of hyponatremia occurring pre operation in senile patients with pituitary adenomas.Methods Preoperative hyponatremia in four senile patients with pituitary adenomas was analyzed in conjunction with reviewing of literature.Results All the four cases with preoperative hyponatremia were the patients with macro adenomas,aged more than 65.The hyponatremia resulted from water retention and could be ameliorated by restriction of water intake and promptly corrected through resection of adenomas.Conclusion hyponatremia in senile patients with pituitary adenomas may be associated with compression of hypothalamus by adenomas and is presented with similar manifestations to syndrome of inappropriate secretion of antidiuretic hormone(SIADH).
