摘　　要：背景:功能性消化不良(FD)的发病机制尚未明确。有研究提示FD患者血浆血管活性肠肽(VIP)水平增高,但胃黏膜局部VIP mRNA的表达情况尚不清楚。目的:研究胃窦黏膜VIP mRNA的表达在FD发病中的作用。方法:采用逆转录聚合酶链反应(RT-PCR)检测30例FD患者和15例正常对照者胃窦黏膜VIP mRNA和B-肌动蛋白(actin)mRNA(内参照)的表达量,计算VIP mRNA与B-actin mRNA表达量的积分光密度比值(V/B)。结果:在FD各亚型中,运动障碍样、溃疡样和非特异性消化不良组的V/B值分别为1.53±0.24、1.27±0.30和1.31±0.11,仅运动障碍样消化不良组与正常对照组(1.24±0.19)相比有显著差异(P=0.002)。结论:运动障碍样消化不良患者胃窦黏膜VIP mRNA表达量显著增多,胃窦黏膜VIP mRNA的表达改变可能参与了运动障碍样消化不良的发病。Background: The mechanism of functional dyspepsia (FD) is still unclear. It has been reported that the level of plasma vasoactive intestinal peptide (VIP) is increased in patients with FD, but the expression of VIP mRNA in the gastric mucosa of these patients is unknown. Aims: To study the effect of VIP mRNA expression in gastric antral mucosa on the pathogenesis of FD. Methods: Using reverse transcriptase polymerase chain reaction (RT-PCR), the expression levels of VIP mRNA and P-actin mRNA, the inside reference, in gastric antral mucosa were quantified in 30 FD patients and 15 normal controls. The ratio of VIP mRNA to β-actin mRNA (V/B) in the integrated optical density was calculated. Results: The ratio of V/B in the different FD subtypes, including dysmotility-like dyspepsia, ulcer-like dyspepsia and non-specific dyspepsia, were 1.53 ±0.24, 1.27 ±0.30 and 1.31 ±0.11, respectively. Only the ratio of V/B in patients with dysmotility-like dyspepsia had statistically significance compared with that in the normal controls (1.24 ±0.19, P=0.002). Conclusions: The expression level of VIP mRNA in gastric antral mucosa increase significantly in patients with dysmotility-like dyspepsia, which suggest that the variation of VIP mRNA expression in gastric antral mucosa may be involved in the pathogenesis of dysmotility-like dyspepsia.
