晚期恶性肿瘤患者化疗前后CD8^+、CD8^+CD28^+、CD8^+CD28^-T细胞的变化观察

摘　　要：目的 :分析 6 4例晚期恶性肿瘤患者化疗前后CD8+ 、CD8+ CD2 8+ 、CD8+ CD2 8- 的变化 ,并探讨其与病情的关系。方法 :采用流式细胞术检测 6 4例晚期恶性肿瘤患者治疗前后T淋巴细胞亚群 ,并与健康体检者作比较。结果 :实验组晚期恶性肿瘤患者化疗前CD4 + 低于对照组 (P <0 0 5 ) ,而化疗前后CD4 + CD8+ 明显低于对照组 (P <0 0 1) ,CD8+ 、CD8+ CD2 8- 明显高于对照组 (P <0 0 1)。治疗后 ,化疗有效者CD4 + 、CD4 + CD8+ 、CD8+ CD2 8+ 升高 (P <0 0 5、P <0 0 1) ,而CD8+ CD2 8- 降低 (P <0 0 5 ) ;化疗无效者CD4 + CD8+ 、CD8+ CD2 8+ 降低 (P <0 0 5、P <0 0 1) ,CD8+ CD2 8- 升高 (P <0 0 1)。结论 :晚期恶性肿瘤患者T淋巴细胞免疫功能低下 ,有效的治疗能提高患者的T淋巴细胞免疫功能 ,临床上检测CD8+ 、CD8+ CD2 8+ 、CD8+ CD2 8- 对恶性肿瘤的治疗和预后有一定的监测作用。Objective:To analyze the changes of CD8 +、CD8 +CD28 +、CD8 +CD28 - before and after chemotherapy and to explore the relationship between their changes and the severity of condition in the patients with advanced malignancies.Methods:T lymphocyte subgroups were detected by the flow cytometry before and after treatment in the patients with advanced malignancies and compared with control group healthy.Results:CD4 + in advanced malignancies was lower than that in control group before chemotheray(P<0 05),while CD4 +/CD8 + was significantly lower than that in control group before and after chemotherapy(P<0 01).CD8 +,CD8 +CD28 - were significantly higher than that in control group before treatment(P<0 01);after treatment,CD4 +,CD4 +/CD8 +,CD8 +CD28 + cell in the effective group were higher than that in pre treatment sample(P<0 05、P<0 01),while CD8 +CD28 - cell was lower than that in pre treatment sample(P<0 05).CD4 +/CD8 +.CD8 +CD28 + cells were lower in the ineffective group than that in pre treatment sample(P<0 05、P<0 01).CD8 +CD28 - cell were higher than that in pre treatment sample(P<0 01).Conclusions:The immunity of T lymphocyte were lower in the patients with advanced malignancies and immunity of the patients can be improved by effective treatment.So clinical detection of the immunity of CD8 +,CD8 +CD28 +,CD8 +CD28 - has certain surveillance function on the treatment and prognosis of the advanced malignancies.
