3292例消化系统恶性肿瘤多重癌发生率分析

摘　　要：[目的]了解原发消化系统恶性肿瘤患者多重癌的发生情况。[方法]对1990年1月～2001年1月11年间在我院住院接受治疗的原发消化系统恶性肿瘤患者3292例进行分析。[结果]3292例原发消化系统恶性肿瘤患者发生多重癌73例(2.22%)。男性56例,女性17例(3.29∶1,P﹤0.01)。同时癌16例(21.92%),异时癌57例(78.08%)(P﹤0.01)。多重癌发生率由高到低依次为原发胰腺癌10.29%(7/68)、食管癌2.73%(16/587)、肠道肿瘤2.40%(22/915)、胃癌2.15%(25/1164)、肝癌1.95%(10/514),除胃癌与肝癌之间无显著差异外,其余之间比较差别均有统计学意义(P﹤0.01)。合计发生部位84处,由高到低依次为肠癌15处(17.86%)、胃癌14处(16.67%)、鼻咽喉癌9处(10.71%)、食管癌8处(9.52%)、肺癌6处(7.14%)、膀胱癌5处(5.95%)、前列腺癌5处(5.95%)、肾癌3处(3.57%)、肝癌2处(2.38%)、睾丸癌2处(2.38%)。[结论]本组原发消化系统恶性肿瘤患者多重癌的发生率为2.22%,涉及消化、呼吸和泌尿系统。原发胰腺癌多重癌发生率最高,肠道肿瘤发生部位最多。[Purpose]To investigate the incidence of the multiple primary cancers in patients with pri-mary digestive system carcinoma.Data of3292patients with primary digestive system carci-nomas who were admitted to our hospital from Jun.1990to Jun.2001were analyzed retrospectively.Seventy-three cases(2.22%)with multiple primary cancers were found in3292patients with primary digestive system carcinoma.Of73cases,56were male,17were female.And16cases(21.92%)had multiple simultaneous carcinomas,57case(78.08%)had multiple asynchronous carcinomas(P〈0.01).The incidence of multiple primary cancer was10.29%(7/68)in the primary pancreas cancer,2.73%(16/587)in the primary esophageal cancer,2.40%(22/915)in the primary intestinal cancer,21.5%(25/1164)in the gastric cancer and1.95%(10/514)in the liver cancer(P〈0.01).There were significant difference between them,except the gastric cancer and liver cancer.The total sites occur-ring multiple primary cancers were84,including intestinal cancer15sites(17.86%),gastric cancer14sites(16.67%),head and neck cancer9sites(10.71%),esophageal cancer8sites(9.52%),kidney cancer3site(3.57%),liver cancer2sites(2.38%)and testis cancer2sites(2.38%).[Conclusion]The inci-dence of multiple primary cancer in patients with primary digestive system carcinoma is2.22%in this series.They involve digestive system,respiratory system and urinary system.The incidence of the multi-ple primary cancer in patients with primary carcinoma of pancreas is the highest,and the most site of multiple primary cancer is in digestive tract cancer.
