骨髓外组织检出海蓝组织细胞的研究

摘　　要：目的　前瞻性研究骨髓外组织检出海蓝组织细胞的特点及意义。方法　对临床上所有进行穿刺及脱落细胞学检查的标本同时做H E染色和瑞氏—姬姆萨染色 ,然后镜检找海蓝组织细胞 ,对被检出海蓝组织细胞的患者再行骨髓穿刺检查 ,确认骨髓中是否也有海蓝组织细胞 ,临床上重点观察其有无肝、脾肿大、皮肤有无出血斑、BPC是否减少。结果　从 5 76 0例被检标本中检出海蓝组织细胞 8例 ,此 8例患者骨髓中未见海蓝组织细胞 ,临床上无肝、脾肿大 ,皮肤无出血斑 ,BPC无减少。结论　骨髓外检出海蓝组织细胞的患者临床表现与“海蓝组织细胞增生症”不同 ,可能为骨髓外局部组织海蓝组织细胞增生的缘故 ,认为骨髓外组织也可分化产生海蓝组织细胞。Objective:To study retrospectively the characteristics and significance of sea blue histocytes detected outside bone marrow. Methods: All specimens from aspiration cytology and detached cytology studies were examined for sea blue histocytes after stained by hematoxylin-eosin (H-E) and Wright-Giemsa. For those with sea blue histocytes found outside BM, BM aspirations were performed to determine whether or not such cells were also in BM. Clinical observations focused on the presence of hepatomegaly, splenomegaly, petechia, and thrombocytopenia. Results: Of the 5760 cases examined, sea blue histocytes outside BM were detected in 8, who had neither sea blue histocytes in BM nor hepatomegaly, splenomegaly or thrombocytopenia.Conclusion: The clinical features of the patients with sea blue histocytes detected outside BM are different from those of "sea blue histocytosis". These cells probably originate from proliferation of sea blue histocytes in local tissue other than BM. It is thought that extra-marrow tissues may also differentiate to produce sea blue histocytes.
