实验性兔眼aPVR睫状体组织病理及超微结构观察

摘  要:目的 探讨前部增生性玻璃体视网膜病变 (anteriorproliferativevitreoretinopathy ,aPVR)睫状体组织病理及超微结构改变 ,为探讨aPVR引起慢性低眼压的机理提供依据。方法 利用培养的同种兔皮肤成纤维细胞制作aPVR引起慢性低眼压的动物模型 ,于术前及术后不同时间点分别观测眼压 ,做组织病理及电镜观察。结果 术后 7、14、2 8、5 6d实验组平均眼压明显低于对照组 (P <0 .0 1)。光镜检查发现实验组术后 14、2 8、5 6d睫状体水肿 ,术后 2 8d及 5 6d睫状体无色素上皮萎缩、缺失。电镜检查发现实验组术后 2 8d和 5 6d睫状体无色素上皮线粒体明显减少 ,细胞内空泡。结论 在aPVR病理状态下 ,睫状膜的增生和收缩引起对睫状上皮的牵拉 ,造成睫状体无色素上皮萎缩和睫状体水肿 ,可能是引起慢性低眼压的主要原因。Objective To study histology and ultramicroscopic structure of ciliary body in anterior proliferative vitreoretinopathy (aPVR),and to provide knowledge about aPVR inducing chronic hypotony.Methods Animal models of chronic hypotony by aPVR were made with cultured homologous dermal fibroblasts on pigmented rabbits.The intraocular pressure (IOP) were taken preoperation and on days 7,14,28 and 56 postoperation.Rabbits were killed on days 14,28 and 56 postoperation respectively,prepared for histology and ultrastructure examination.Results The average IOP of experimental group was lower than that of control group on days 7,14,28 and 56 significantly(P<0.01).Microscopic examination showed edema of the ciliary body on days 14,28 and 56,and atrophy or absence of the non-pigmented ciliary epithelium on days 28 and 56 postoperation in experimental group.Electronic microscopy showed that the amount of mitochondrions decreased and there were many vacuoles in the non-pigmented ciliary epithelium in experimental group 28 and 56 days postoperation.Conclusions The epiciliary membrane in aPVR dragged the ciliary body,made atrophy of non-pigmented epithelium and edema of ciliary body,further caused the hypotony.
