奥曲肽合并高能聚焦超声热疗治疗晚期胰腺癌疗效初探

摘　　要：目的 试用奥曲肽合并高能聚焦超声热疗治疗晚期原发性胰腺癌,评价其疗效和临床受率情况。方法 使用高能聚焦超声肿瘤治疗机用输出功率200～300 W对肿瘤进行热疗,每次治疗时间约为1 h,间隔48 h重复治疗。同时给予善宁(Octreotide)100 μg皮下注射,3次/d,使用3周,停1周。用至疗效有进展证据时停药。结果 6例晚期原发性胰腺癌,1例完全缓解,3例部分缓解,1例微效,1例稳定,临床受益疗效6例患者中,有2例Ⅰ、Ⅱ级疼痛消失,2例Ⅲ级疼痛减轻至Ⅱ级,1例Ⅳ级疼痛减至Ⅱ级,1例疼痛无明显减轻。结论 奥曲肽合并高能聚焦超声热疗,初步疗效非常显著,不失为一种有意义的探索。Objective: To evaluate the clinical effect and clinical benefit(%) of Octreotide & high-intensity focused ultrasound(HIFU) thermotherapy in the treatment of late period primary pancreatic carcinoma. Methods: 1 thermotherapy on tumor was repeated in HIFU therapy machine with output power 200 -300 W every 48. Meanwhile, subcutaneous injection octreotide 100 ug q8 h, 1 week pause every 3 weeks. until progress. Results: 1 case was CR, 3 cases PR, 1 case SD, and 1 case PD in total 6 cases primary pancreatic carcinoma.Ⅰ-Ⅱ grade of pain disappeard in 2 patients, Ⅲ grade of pain relieved to Ⅱ grade in 2 patients, Ⅳ grade of pain to Ⅱ grade in 1 patient, no obvious in 1 patient. Conclusion: These results show the significant clinical effects of octreotide and high-intensity focused ultrasound thermotherapy in the treatment of late period pancreatic carcinoma.
