重症肌无力患者血清sICAM-1、IL-18及IFN-γ水平的监测

摘　　要：目的 探讨可溶性细胞间黏附分子-1(sICAM-1)、白细胞介素-18(IL-18)及干扰素-γ(IFN-γ)与重症肌无力(MG)的关系。方法 应用酶联免疫吸附试验(ELISA)法动态监测45例不同类型的MG患者(MG组)和25名健康者(NC组)血清sICAM-1、IL-18及IFN-γ水平,并对MG患者病情按许贤豪评分法进行量化。结果MG患者血清sICAM-1及IL-18水平均明显高于NC组(P<0.01),经皮质类固醇治疗后两者水平均明显下降(P<0.01);MG组和NC组血清均未检测到IFN-γ;病程<6个月组患者sICAM-1水平明显高于病程>1年组(P<0.01);MG患者血清IL-18水平与许贤豪绝对评分呈明显正相关(r=0.767)且具有统计学意义(P<0.01)。结论 血清sICAM-1及IL-18水平可望成为观察MG患者疾病活动性、病情和免疫疗效有一定意义的参数。Objective To explore the relationship of soluble intercellular adhesion molecule-1 (sICAM-1), interleukin-18(IL-18) and interferon-γ(IFN-γ) in serum of myasthenia gravis(MG). Methods The levels of sICAM-1,IL-18 and IFN-γ were detected by ELISA assay in 45 MG patients while 25 healthy donors were served as a normal control (NO. MG group was subdivided into subgroups according to their absolute scores rated by XU Xian-hao score system. Results The levels of sICAM-1 and IL-18 of MG were significantly higher than those of NC group( P <0. 01) and were both reduced after immunochemotherapy;IFN-γ can not be determined in MG and NC groups;The level of sICAM-1 in MG patients with in course six months was significantly higher than one year ( P <0. 01); There was positive correlation between the serum level of IL-18 and the myasthenic scores rated by XU Xian-hao score system( r =0. 767). Conclusions Serum sICAM-1 and IL-18 are expected to be referable reliable parameters in evaluating the disease activity, severity and therapeutic efficacy.
