胃癌组织环氧合酶-2表达及其与临床病理学特征的关系

摘　　要：目的　观察胃癌组织COX 2表达状况及其与临床病理学特征的关系。　方法　应用免疫组化法检测胃黏膜组织COX 2的表达。　结果　 2 2例慢性浅表性胃炎 (对照组 )中仅 2例COX 2低表达阳性(9 .1% )。 32例胃癌中发现COX 2表达阳性 2 2例 (6 8.7% )。其中伴淋巴结转移者COX 2表达阳性率 80 .9% (17/ 2 1) ,高于不伴淋巴结转移者 4 5 .4 % (5 / 11) (P <0 .0 5 )。而侵犯浆膜及以外者 (73.9% )与未侵犯浆膜者 (5 5 .6 % )、进展期胃癌 (71.4 % )与早期胃癌 (6 0 % )COX 2表达阳性率比较无显著性差异 (P >0 .0 5 )。低未分化型癌COX 2表达阳性率 (81.8% )虽高于黏液性癌和高中分化型腺癌 ,但三组间比较无显著性差异 (P >0 .0 5 )。　结论　胃癌组织中存在COX 2过度表达 ,且主要与淋巴结转移相关。Objective To investigate the status of cyclooxygenase-2 (COX-2) expression in gastric carcinorna and its reatinship to clinicopathologic features. Methods COX-2 expression in gastric mucosa was detected immunohistochemically. Results COX-2 expression was found to be positive only in 2 out of 22 cases of superficial gastritis without macroscopic lesions(9.1%). While COX-2 expression was detected in 22 out of 32 cases of gastric carcinoma, and the positive rate was 68.7%.Among these 32 cases of gastric carcinoma,the COX-2 expression rate was significantly higher in the cases with lymph node involvement(80.9%) than those without lymph node involvement(45.4%). However, there was no significant difference between early gastric carcinoma and advanced gastric carcinoma ,as well as between the cases with serosa invasion and those without serosa invasion in COX-2 expression. In terms of Pathologic type of gastric carcinoma, COX-2 expression was higher in low-differentiated adenocarcinoma(81.8%) than well-differentiated adenocarcinoma (60.0%) and mucous carcinoma(63.4%), but there was no statistical difference among them. Conclusion COX-2 is overexpressed in gastric carcinoma tissues,and is related to lymphatic metastasis.
