肉毒毒素注射结合上田法和中医疗法在小儿脑性瘫痪中的应用研究

摘　　要：目的　探讨治疗脑性瘫痪 (CP)的有效方法。方法　 64例CP患儿 ,随机分为治疗组和对照组 ,每组各 3 2例。在中医疗法 (推拿、头针、穴位注射 )基础上 ,治疗组采用上田法和肉毒毒素治疗 ,对照组采用Bobath法治疗。采用Koman医师评分法 (PRS)判断BTX A注射后疗效 ,两组患者治疗前及治疗 6个月后以小儿CP粗大运动量表 (GMFM)和改良Ashworth痉挛量表进行评定。结果　两组治疗前、后组内相比 ,肌痉挛均明显改善 ,P <0 .0 1,运动功能均有显著提高 ,P <0 .0 1。两组治疗后组间相比 ,治疗组疗效明显优于对照组 ,P <0 .0 1。结论　肉毒毒素注射结合上田法和中医疗法治疗小儿CP具有十分重要的意义 。Objective To explore the effective treatment regimen to improve the function of children with cerebral palsy (CP). Methods Sixty four patients with CP were divided randomly into two groups:a treatment group and a control group. All the patients were treated with Chinese massage, head accupuncture and accupuncture point injection, in addition, the patients in the treatment group were also treated with botulinum toxin A( BTX A) and Wuida approach, while those in the control group with Bobath approach. The effect was evaluated by the physician rating scale (PRS), gross motor function measure (GMFM) and modified Ashworth scale before and after treatment. Results The motor function was improved significantly after treatment of patients in both groups. Spasticity was also improved significantly in all the patients. A comparison between the two groups revealed that the curative effect of the treatment group was significantly superior to that of the control group( P <0.01). Conclusion The results suggested that the application of BTX A injection, Wuida approach and traditional Chinese medicine therapy in combination is of great importance for CP, especially in improving the patients' motor function and balance ability.
