摘 要:目的 :探讨急性脑梗塞患者血浆内皮素 (ET)、降钙素基因相关肽 (CGRP)的水平变化 ,以阐明生物活性肽与缺血性卒中之间的关系及作用机制。方法 :应用放射免疫分析 (RIA)检测 36例患者和 32例正常对照血浆ET、CGRP的浓度。结果 :急性早期患者血浆ET(10 3 11± 2 8 2 7)ng/L、CGRP(30 5 4± 13 4 6 )ng/L与正常对照组 (4 7 5 9± 7 0 9)ng/L和 (4 8 6 7± 15 30 )ng/L两者相比较均有显著性差异 (p <0 0 1)。而早期与恢复期患者对照比较亦有显著性差异 (p <0 0 1)。血浆ET升高和CGRP下降在病程短病灶大及伴高血压的患者较病程长病灶小及不伴高血压者更明显。大梗塞灶血浆ET、CGRP水平呈负相关 (r =- 0 6 82 ,p <0 0 5 )。其ET/CGRP比值随着病程延长而下降 (p <0 0 1)。 结论 :急性脑梗塞患者存在着ET与CGRP对脑血管内皮功能影响 。Objective To illustrate the relationship between vasoactive peptides and ischemic cerebrovascular diseases through investigation of the changes of the plasma levels of ET and CGRP in patients with acute cerebral infarction. Methods Plasma ET and CGRP concentrations were measured with RIA in 36 patients with acute cerebral infarction and 32 controls. Results Plasma ET (103 11±28 27ng/L) and CGRP(30 54±13 46ng/L) levels in the patients within 7 days after onset of the disease were significantly different from those in the controls (ET 47 59±7 09ng/L, CGRP 48 67±15 30ng/L) ( p <0 01). Differences between the levels of these vasoactive peptides in the early stage of the disease (within 7 days after onset) and during convaleseance (over 25 days) were also significant ( p <0 01). The increase of plasma ET content and decrease of CGRP content were more marked in those patients with acute, larger-sized lesion complicated with hypertension than in those with chronic, smaller-sized lesion and no hypertension. In patients with large-sized lesions, the plasma ET levels were negtively correlated to the plasma CGRP levels ( r=0 682,p <0 05). Plasma ET/CGRP ratios decreased gradually after the onset (early days vs convalescence, p <0 01). Conclusion Plasma ET and CGRP participated in the whole patho-physiologic process of acute cerebral infarction. Disturbance of the normal balance between these two peptides greatly worsened the disease process. Dynamic measuements of their plasma concentrations helped to predict the severity of the disease.
