幽门螺杆菌感染导致胃黏膜黏蛋白的异常表达

摘　　要：目的　探讨胃黏膜幽门螺杆菌 (H .pylori)感染与黏蛋白表达的关系。方法　研究对象来自 4 6例外科手术的胃癌患者 ,其中 39例癌旁组织 ,另有 10例正常人胃黏膜。采用H .pylori的检测采用Warthin Starry银染法 ,黏蛋白的检测采用免疫组化方法。结果　 2 1例H .pylori感染者中有 14例 (6 6 7% )癌旁黏膜中呈现MUC2黏蛋白的表达 ,18例无H .pylori感染者黏膜中有 6例 (33 3% )呈现MUC2黏蛋白的表达 ,两组比较差异显著(P <0 0 5 )。 2 1例H .pylori感染者中有 7例 (33 3% )癌旁胃黏膜组织中MUC1黏蛋白呈强阳性表达 ( ) ,而 16例胃癌H .pylori感染阴性者中有 12例 (75 % )癌旁胃黏膜组织中MUC1黏蛋白呈强阳性表达 ,两组比较有显著性差异 (P <0 0 5 )。2 1例H .pylori感染者中有 7例(33 3% )癌旁胃黏膜组织中MUC6黏蛋白呈阳性表达 ,而 16例H .pylori感染阴性者中有11例(6 8 8% )癌旁胃黏膜组织MUC6黏蛋白呈强阳性表达 ,两组比较也有显著性差异 (P <0 0 5 )。 2 0例H .pylori感染者中有 5例(2 5 % )癌旁胃黏膜组织MUC5AC黏蛋白呈强阳性表达 ,而 14例H .pylori感染阴性者中有 10例 (71 4 % )癌旁胃黏膜组织MUC5AC黏蛋白呈强阳性表达 ,两组比较有显著性差异 (P <0 0 1)。胃癌患者H .Objective The study was to explore the relationship between Helicobacter pylori ( H.pylori ) infection and aberrant mucin expression in gastric mucosa. Methods H.pylori infection was diagnosed by Warthin Starry staining method, and different kinds of mucin were detected using the immunohistochemical method. Results Positive staining of MUC2 mucin was found in 14 out of 21 patients with mucosa positive for H.pylori ( 66 7%), whereas only 6 cases showed MUC2 mucin expression in 18 H.pylori negative patients (33 3%) ( P <0 05). 7 cases had strong expression of MUC1 () in 21 H.pylori positive patients (33 7%), wheareas 16 cases had strong MUC1 expression in 16 H.pylori negative patients (75%) ( P <0 05). Positive staining of MUC6 was found in 7 out of 21 H.pylori positive infection patients (33 3%), 11 cases showed strong positive staining in 16 H.pylori negative patients (33 3%) ( P <0 05). Strong expression of MUC5 () was shown in 5 out of 20 H.pylori positive infection patients (25%), wheareas stong MUC5 expression was observed in 10 out of 14 H.pylori negative patients (71 4%) ( P <0 01). No correlation was found between MUC3 and H.pylori infection ( P >0 05). Conclusion H.pylori infection may alter the expression of some mucin genes in pericancerous gastric mucosa and destroy the gastric mucosa barrier
