巩膜小切口白内障摘除术上方与颞侧切口的比较

摘　　要：目的 :探讨巩膜小切口白内障摘除术采取不同部位切口的疗效和技术。方法 :对老年性白内障 76眼随机分成两组 ,分别采用上方与颞侧巩膜隧道切口 ,行 7.0mm弦长小切口白内障摘除术 ,植入光学部直径 6 .5mm后房型人工晶体。结果 :颞侧切口组 37眼术后 35眼 (94 .6 % )产生循规性角膜散光变化 ,上方切口组 39眼中 34眼(87.2 % )产生逆规性角膜散光变化 ,即全部病例中 90 .8%出现逆切口的角膜散光变化。上方切口组早期角膜散光绝对值大于颞侧切口组 (P <0 .0 5 )。随术口愈合散光度数逐渐减少 ,两组间的角膜散光差异也减少 ,术后 3月后差异无显著性 (P >0 .0 5 )。术后两组间的各期视力差异均无显著性 (P >0 .0 5 )。结论Objective:To evaluate the therapeutic effect of superior versus temporal incision in small scleral incision in cataract surgery. Methods: Fifty six eyes undergoing 7.0 mm small scleral incision cataract surgery were assigned randomly to the superior incision group and temporal incision group. Rigid 6.5 mm PMMA PCIOLs implantation were performed. Results: Postoperatively,the superior incision group showed against the rule astigmatic changes,whereas with the rule astigmatism was seen in the temporal group. The superior incision group showed more astigmatic effect than the temperal incision group 1 week and 1 month( P <0.05).The amount of the astigmatism reduced while the incision healed up. No significant intergroup difference was observed in the amount of astigmatism after 1 month ( P >0.05) and in visual acuity postoperatively ( P >0.05,chi square test). Conclusion: Superior and temporal small scleral incisions in cataract surgery are comparable in the long term efficacy of visual rehabilitation and astigmatism induction.
