摘 要:目的 探讨不同眼外肌手术刺激引起的胃电图变化。方法 应用体表胃电描记法对眼科病房 1 63例行双眼外直肌后徙术或双眼内直肌后徙术患儿的胃电活动进行监测。结果 斜视儿童内直肌或外直肌因手术刺激引起胃电图变化的主要表现为主功率增加 ,内直肌组 3 7例主功率差值增加 :胃窦 63 5 9μV± 1 2 8 2 2 μV、胃体 78 2 4μV± 1 43 85 μV ;外直肌组 1 2 6例主功率差值增加 :胃窦 5 7 3 5 μV± 1 89 60 μV、胃体 64 95 μV± 1 2 9 94μV ,内直肌组主功率差值增加较外直肌组程度更为明显。与此同时 ,眼 -胃反射发生率内直肌组 ( 4 3 2 4% )也略高于外直肌组 ( 4 0 47% ) ,但其差别均无明显统计学意义。胃电图记录与临床表现基本一致。结论 胃电图是监测不同眼外肌手术眼 -胃反射的一种可以量化的客观检查方法 ,内直肌与外直肌后徙手术时的胃电图未呈现明显差别。A comparision of changes of electrogastrogram in surgery of different extraocular muscles. Ye Ying-fu, Tang Hong, Qiao Tong, et al. Department of Ophthalmology, Wuhan Children′s Hospital, Wuhan 430016 Objective To compare the changes of electrogastrogram (EGG) caused by surgical stimulation of different extraocular muscles. Methods Examination of gastroelectric activity of 163 cases of strabismus children submitted to binocular lateral rectus recession or binocular medial rectus recession by means of body surface eletrogastrography. Results The EGG change caused by surgical stimulation of extraocular muscles were mainly an increase of action potential (AP), the AP in medial rectus surgery (37 cases) in creased (63.59±128.22μV) on antrum and (78.24±143.85μV) on stomach body, in lateral rectus surgery (126cases) increased (57.35±189.60μV) on antrum and (64.95±129.94μV) on stomach body. The increase of AP in medial rectus group was somewhat larger than the lateral rectus group. Meanwhile the rate of oculogastric reflex (OGR) in medial rectus group (43.24%) was also somewhat higher than the lateral rectus group (40.47%), but both the difference of change of AP and the rate of OGR between two groups were statistically not significant. The clinical manifestations of 2 groups also showed no obvious difference. Conclusion EGG is an objective and quantitative method to estimate the OGR in extraocular muscle surgery. No obvious difference of EGG was found between binocular lateral rectus recession and binocular medial rectus reession.
