摘  要:目的 探讨VEGF和Ki- 6 7表达与星形细胞肿瘤恶性程度、微血管密度 (MVD)、瘤细胞增殖以及病人预后等方面的关系 ,评估两者在病理诊断和临床治疗中潜在的应用价值。方法 采用免疫组化S -P法 ,分析 4 8例脑星形细胞肿瘤标本中VEGF蛋白和Ki- 6 7的表达 ,并用FⅧ -RA标记血管内皮细胞以计算MVD。结果 VEGF阳性率在低、高度恶性星形细胞肿瘤中分别为 2 6 .0 9%、6 4 .0 0 % ;在低度恶性星形细胞肿瘤中Ki- 6 7LI和MVD分别为 8.89± 5 .80和 6 .6 9± 4 .81,但在高度恶性肿瘤中分别为 17.4 9± 13.13和 2 3.6 2± 6 .78,其差异具有显著性 ;术后生存两年以上者 ,VEGF表达、MVD和Ki- 6 7LI明显低于术后生存两年以下者 ;相关分析表明VEGF蛋白表达、MVD与Ki- 6 7LI间存在相关性。结论 VEGF可促进肿瘤血管形成和恶性进展 ,对病人的预后产生影响 ;同时定量检测MVD和Ki- 6 7,对星形细胞肿瘤病理分级和预后判断有一定帮助。Objective To evaluate the relationship of VEGF and Ki-67 expression with the pathologic grades, microvessels density, and the prognosis in patients with astrocytic tumors. Methods 48 astrocytic tumor tissues were investigated for VEGF, Ki-67 and FⅧ-RA using S-P immunohistochemical staining. Results There was significant difference of the VEGF expression level between low and high grades astrocytic tumors, in which the positive rate were 26.09% and 64.00% respectively. In high grades (Ⅲ|Ⅳ) astrocytic tumors, Ki-67LI and MVD was also higher than those in low grades (Ⅰ、Ⅱ) astrocytic tumor.In patients with less than two years survival, VEGF positive expression rate, MVD and Ki-67LI are higher than those patients with more than two years survival. Positive correlation also was found between these indexes. Conclusion VEGF can promote tumor angiogenesis and may have effect on the prognosis of the patients. These two indexes, tumor cell proliferation and MVD, can be used to evaluate tumor pathological grades and the prognosis of patients with astrocytic tumor,if they are investigated at the same time.
