椎基底动脉供血不足患者TCD改变与颈椎不稳

摘　　要：目的 :探讨椎基底动脉供血不足 (VBI)经颅多普勒超声 (TCD)与颈椎不稳的关系。方法 :对 6 4例VBI患者和 30例正常人进行颈椎X线检查和TCD检查 ,并进行相关性分析。结果 :VBI组共有 5 4例 (84 .3% )颈椎不稳 ,TCD异常 5 6例 (87.5 % ) ,两项检查均为异常者 5 3例 ,均为正常者 6例 ,两种检查结果异常一致率 82 .8% (5 3/ 6 4 ) ;正常对照者组有 4例 (13 3% )颈椎不稳 ,TCD异常 5例(16 6 % ) ,经 χ2 检验 ,P <0 .0 1,两项检查均为异常者 4 / 30例 ,均为正常者 2 5 / 30例 ,两种检查结果正常一致率 83.3% (2 5 / 30 )。结论 :颈椎不稳与TCD异常有密切关系 。Objective:To investigate the relationship between transcranial doppler (TCD) and cervical instability in patients with vertebrobasilar insufficiency (VBI). Methods Cervical vertebrae radiographs and TCD were done in 64 patients with VBI and and 30 normal controls ,and the correlative analysis was performed. Results:In VBI group,cervical instability was found in 54 patients (84.3%) and abnormal TCD in 56 ones (87.5%).Both cervical vertebrae radiographs and TCD were found abnormal in 53 patients and normal in 6 ones.The abnormal coincidence rate of the two methods was 82.8%.In control group,cervical instability was found in 4 patients (13.3%) and abnormal TCD in 5(16.6%). Both cervical vertebrae radiographs and TCD were found abnormal in 4 out of 30 subjects and normal in 25 out of 30 ones.The normal coincidence rate was 83.3%.Conclusion Cervical instability might be closely related with the abnormal TCD.It might be one of the cause inducing recurrent vertigo in patients with VBI.
