大肠癌患者血浆和组织胃泌素的检测

摘　　要：目的：探讨胃泌素在大肠癌生长中的作用。方法：应用放射免疫法检测４０ 例大肠癌患者和１０ 例大肠腺瘤患者血浆胃泌素、组织胃泌素和相应的癌旁正常粘膜组织胃泌素，及４０例正常对照组的血浆胃泌素。结果：大肠癌组血浆胃泌素无明显增高，但血浆胃泌素浓度与肝转移呈显著正相关；同时发现大肠癌和腺瘤的组织胃泌素水平明显高于癌旁正常肠粘膜（Ｐ＜ ０．０５），且组织胃泌素水平与大肠癌的Ｄｕｋｅｓ分期及转移呈显著正相关。结论：大肠癌组织具有自分泌胃泌素及促肿瘤生长作用，且胃泌素表达的增加很可能发生在肿瘤的中、晚期，因而可作为判断大肠癌患者预后的重要指标。Objective:To evaluate the hypothesis that gastrin might function as a local growth factor in colorectal neoplasms.Methods:Blood samples were taken from 40 cases colorectal cancer 、10 cases adenomatous polyposis and 40 healthy controls.Tumors and normal tissue were obtained from 40 patients undergoing surgery for colorectal cancer.Gastrin in blood samples and tissue was measured by radioimmunoassay.Results:Plasma gastrin levels were no higher in subjects with colorectal cancer or adenomas than those of controls,but there was significant correlation between plasma gastrin levels and liver metastasis.Besides,there was significant difference among colorectal cancers,adenomatous polyposis and controls.In colorectal cancer subjects,there were no significant correlation between tissue gastrin and Dukes staging,liver metastasis.Conclusion:The results indicat that gastrin may play a stimulating role in the growth of tumor and serve an important indicator of prognosis of colorectal cancer.
