摘  要:目的通过动物角膜碱烧伤模型,观察烧伤后不同时期及烧伤后用激素治疗其血清特异性抗体水平及组织病理的变化。方法新西兰白兔16只制做单眼角膜中央部中度碱烧伤模型并随机分为2组,烧伤组、激素治疗组(烧伤后口服强的松10mg 8wk)。制备正常及碱烧伤角膜蛋白提取液,用酶联免疫吸附试验(ELISA)检测每组兔不同时期血清抗角膜变性蛋白抗体水平,并取不同时期的角膜做光镜观察。结果角膜碱烧伤后机体产生特异性抗体,2wk时升高明显、5~6wk达高峰,之后下降。激素治疗后抗体水平总体下降,但2wk仍有升高、4wk达高峰、5wk急剧下降、8wk时明显低于烧伤前水平。大体观察及光镜结果显示:激素组与烧伤组比较,上皮愈合好、基质纤维排列整齐、炎症细胞浸润轻、新生血管少。结论激素治疗角膜碱烧伤可在相当程度上阻断机体体液免疫反应过程。Purpose To observe the changes of serum specific anti-denatured corneal antibody level and histopathology in different stages and oral steriod treatment after burns, animal model of comeal alkali bums was used. Methods 16 male New Zealand rabbits, with alkali burns in right eyes were randomly divided into 2 groups including: the burned group; steroid treatment group (oral prednisone 10mg/d) .After making the normal or burned corneal supernatant, we detected the level of serum specific antibody in every group by ELISA test and observed the corneal structure in different stages after alkali burns by light microscopy. Results The body produced specific antibody after corneal alkali burns. The level of antibody significantly increased at the 2nd week, peaked at the 5 - 6th week, and then decreased. However, the level of antibody increased less in steroid treatment group compared with the burned group . The results of light microscopy showed , the corneal epithlium recovering better, the fibre of corneal stroma arranging better, inflammative cells infiltrating less and neovascule forming less in steroid treatment group compared with the burned group. Conclusion Steroid treatment in treating corneal alkali burns can block the humoral immune reaction to a great extent and is a feasible and effective method.
