影响正视眼孔源性视网膜脱离手术效果的危险因素

摘　　要：目的对影响正视眼孔源性视网膜脱离可能存在的危险因素进行探讨。方法本院2000年11月至2002年10月住院治疗的正视眼孔源性视网膜脱离的临床资料进行危险因素的Logistic统计分析。结果正视眼孔源性视网膜脱离共363例367眼,304眼一次手术成功,一次手术成功率为82.8%。PVR(P<0.01)、手术方式、不完全玻璃体后脱离、外伤史、裂孔类型、内眼手术史(P<0.05)为影响手术效果的危险因素。结论影响正视眼孔源性视网膜脱离手术效果的危险因素有PVR严重程度、玻璃体状态、裂孔类型以及外伤和内眼手术史等,手术前应当重视这些因素以提高手术成功率。Purpose To study the risk factors affecting surgical outcome of emmetropic rhegmatogenous retinal detachment ( RRD). Methods The data on patients on whom operations were performed for enimetropic RRD in our department from November 2000 to October 2002 were collected . Risk factors affecting surgical outcome were analyzed by Logistic regression.Results Complete data on 367 eyes in 363 patients were available .The retina was successfully reattached with a single operation in 304 eyes. Retinal reattachment with a single operation rate was 82. 8%. Significant risk factors included proliferative vitreoretinopathy (PVR) ( P < 0.01), type of operation, incomplete post vitreous detacliment, history of trauma, type of tears , and history of intracolar surgery ( P < 0.05) . Conclusion This study has shown that the significant risk factors affecting surgical outcome of emmetropic RRD are the severity of PVR, status of vitreous body, type of teal's, history of trauma and intraocular surgery. Greater emphasis should be given to those risk factors before operation so as to improve surgical outcom.
