摘 要:目的研究血清游离态前列腺特异性抗原f-PSA与总PSA比值(f-PSA/t-PSA)在前列腺良、恶性疾病鉴别诊断中的应用价值。方法采用化学发光法检测67例未经治疗的前列腺癌(PCa)患者、193例良性前列腺疾病(BPH,AP,CP)患者和60例健康者血清中的t-PSA和f-PSA,并计算f-PSA/t-PSA比值。结果相较于其他两组,前列腺癌组的t-PSA最高,每两组之间均存在明显的统计学差异(P<0.01)。当t-PSA在4.0-10.0μg/L时,良性前列腺疾病患者t-PSA的数值和前列腺癌患者t-PSA的数值无明显差别(P>0.05)。在f-PSA/t-PSA比值为0.13时,结合病理诊断,灵敏度为66.6%,特异性为84.4%。健康对照组全部处于f-PSA/t-PSA>0.13这个区间,前列腺良性疾病组有168例处于这个区间,前列腺癌组只有5例处于这个区间,三组数据有明显差异(P<0.05)。结论以f-PSA/t-PSA=0.13作为临界值判断点,f-PSA/t-PSA比值将大大提高对前列腺良、恶性疾病诊断的特异性,尤其t-PSA在4.0-10.0μg/L范围内更有意义。Objective To study the value of serum free prostate specific antigen f-PSA and total PSA ratio(f-PSA/t-PSA) in differential diagnosis of benign and malignant prostate diseases. Methods Serum t-PSA and f-PSA in 67 patients with untreated prostate cancer(PCa), 193 patients with benign prostate disease(BPH, AP, CP) and 60 healthy people were detected by chemiluminescence method, and the ratio of f-PSA/to t-PSA was calculated. Results Compared with the other two groups, the prostatic carcinoma group had the highest t-PSA, and there was a statistically significant difference between the two groups(P<0.01). When t-PSA was at 4.0-10.0 μg/L, there was no significant difference in t-PSA between patients with prostatic carcinoma and patients with benign prostatic diseases(P>0.05). When the f-PSA/t-PSA ratio was 0.13, combined with pathological diagnosis, the sensitivity was 66.6%, the specificity was 84.4%. The healthy control group was all in the f-PSA/t-PSA>0.13 interval, prostate benign disease group had 168 cases in this interval, prostate carcinoma group had only 5 cases in this interval, there was significant difference between the data of three groups(P<0.05).Conclusion F-PSA/t-PSA=0.13 is regarded as the critical point, and the f-PSA/t-PSA ratio will greatly improve the specificity of the diagnosis of benign and malignant prostate diseases, especially when t-PSA is in the range of 4.0-10.0μg/L, it has more significance.
