血清CyPA测定在评估急性胰腺炎严重程度中的意义

摘　　要：目的探讨血清亲环素(CyPA)的测定在急性胰腺炎(AP)严重程度评估中的意义。方法检测AP患者起病48h内血清CyPA水平,起病72h内血清C反应蛋白(CRP-72h),收集病人的病历资料(CT﹑APACHⅡ评分﹑有无器官衰竭等),分析CyPA水平在评估AP疾病严重程度的意义。结果(1)重症胰腺炎(SAP)CyPA及CRP-72h水平均升高(P<0.001),血清CyPA与CRP-72h变化一致,两者之间有相关性(r=0.754,P<0.001)。(2)与无器官功能衰竭组比较,器官衰竭组血清CyPA水平更高,两者之间有显著差异(P<0.001)。与无胰腺坏死组比较,胰腺坏死组血清CyPA水平更高,两者之间有显著差异(P<0.001)。结论SAP血清CyPA表达水平显著升高,与疾病的严重程度正相关。Objective to investigate significance of serum cyclophilin(CyPA) in severity assessment of acute pancreatitis(AP). Methods detect serum CyPA level within 48 h and C reaction protein(CRP-72 h) within 72 h of AP patients, collect patient’s medical records(CT, APACH II scores, organ failure, etc.), analyze significance of CyPA level in evaluation of AP severity. Results(1) CyPA level and CRP-72 h of severe acute pancreatitis(SAP) increased both(P < 0.001), and serum CyPA and CRP-72 h has same changes, there was correlation between them(r=0.754, P < 0.001).(2) Comparee with non organ failure group, serum CyPA level of organ failure group was higher, difference was significant between them(P< 0.001). Compared with non pancreatic necrosis group, serum CyPA level was higher in pancreatic necrosis group, difference was significant between them(P< 0.001). Conclusion CyPA level expression in SAP serum is significantly increased, which is positively correlated with severity of disease.
