摘　　要：目的　分析影响颅咽管瘤手术全切除的因素。方法　选择经翼点入路显微手术切除的颅咽管瘤患者 63例 ( 1～ 2级切除 46例 ,3～ 5级切除 17例 ) ,对两组患者的临床资料和病理学资料作统计学分析。结果　临床资料方面 ,肿瘤大小、囊变、肿瘤部位 (鞍上、鞍内鞍上 )两组差异有显著性 (P <0 0 5 )。病理学特征方面 ,肿瘤钙化、病理分型两组差异有显著性 (P <0 0 5 ) ,而脑组织浸润、纤维化程度方面两组差异无显著性 (P >0 0 5 )。结论　肿瘤大小、部位、钙化程度及病变的病理学类型是影响手术全切除的主要因素。Objective To study the factors influencing the resectability of craniopharyngioma. Methods Pathological and clinical data of 63 patients underwent surgery for craniopharyngioma using pterional approaches were analyzed. Results Clinical data showed that there was significant difference between two groups in tumor size,cyst components and tumor location(purely suprasellar or suprasellar with an intrasellar component). Pathological feature revealed that the degree of calcification and pathological subtype were significantly different between two groups, while brain invasion and the presence of fibrosis were not significant different between the two groups. Conclusion Tumor size,location, the degree of calcification and pathological subtype are the main factors influencing total resection of craniopharyngiomas.
