摘  要:目的 研究不同类型脑血管病急性期血清促红细胞生成素 (Epo)的含量与颅脑影像改变及病程的关系。方法 采用放射免疫分析法 (RIA)检测 4 0例脑梗死 (CI)和 38例脑出血 (CH)患者 (病程在 1~ 10天内 )血清Epo含量 ,并与正常组 15例对照。结果  ( 1)脑梗死组和脑出血组患者血清Epo含量均明显高于对照组 (P <0 .0 1)。 ( 2 ) 2组脑血管病患者的血清Epo含量与急性期病程天数无相关性。 ( 3)Pearson相关分析显示 ,血清Epo含量与脑梗死组的影像测定梗死面积和脑出血组的影像测量出血量呈明显负相关 (分别为r=- 0 .6 4 0和r=- 0 .36 1;P均 <0 .0 5 )。结论  ( 1)脑梗死和脑出血病例的血清Epo含量较正常对照组明显增高 ,并与急性期病程天数无相关性 ,说明其增高在脑血管病急性期是相对稳定的。 ( 2 )脑梗死和脑出血病例的血清Epo含量增高与影像学测定的病损范围大小 (脑梗死面积和脑出血量 )均呈负相关 ,说明血清Epo含量增高 ,可能不是脑血管病的病理性损伤的直接结果 。Objective To detect the content of serum erythropoietin(Epo) in the different kinds of acute cerebral vascular diseases, and to investigate the correlation between the changes of Epo and the cerebral infarction area,and cerebral hemorrhage by CT,and the course of cerebral vascular diseases.Methods The serum specimens of 78 cases of acute cerebral vascular diseases were collected within 10 days of course of illness, of which 40 cases were cerebral infarction (CI group), and 38 cases were cerebral hemorrhage (CH group). The control group consisted of 15 patients. The Epo content in serum was detected by radioimmunoassay (RIA).Results (1)Epo content of the CI group and CH group was obviously higher than that of the control group (P<0.01). (2)Epo content of CI group and CH group was not correlated to the days of the acute period.(3) According to Pearson analysis Epo content of CI group and CH group was negatively correlated to the infarction area and the hemorrhage volume (r=-0.64; r=-0.361, P<0.05). Conclusion (1) Serum Epo content of CI group and CH group was significantly higher than that of control group, but there were no changes from 1d to 10d,which suggests that the increase of Epo content is relatively stable in acute cerebral vascular disease.(2)The increase of serum Epo content in CI group and CH group was negatively correlated to the damage area detected by CT (CI area and CH volume),which suggests that the increase of serum Epo content is not the direct outcome of pathological damage of the cerebral vascular diseases, which is likely an outcome of stress response to pathological change in cerebral vascular disease.
